We are very certain Dudley is having a disc episode. He had one last year so this will be his 2nd time. This evening I noticed he had a reluctance to walk, arched back, tight tummy and he keeps losing his balance in his rear legs. He also made a soft moan of discomfort when I picked him up. He is still able to walk and wag his tail but I know this is still an episode. unfortunately the vet was closed for the evening and I was forced to make a judgment call on whether to start him back on his Pred, Tramadol and Pepcid. I still had some of his meds left over from his 1st episode. I also crated him immediately. Did I do the right thing? should I have waited to speak to his Vet/Neurologist before giving him his steroid? Im calling them first thing in the morning and hoping to get him in there on Monday. We cant afford to take him to the emergency Vet and don't trust them....last time they gave us a false diagnosis and put a cast on his leg and told us it was broken...later we found out it was not broken and he had IVDD. Any reply will be greatly appreciated!! thanks so much...dot
i should probably mention that we used your site last year and know the drill.....

I don't know if starting the meds was right or wrong, but I probably would have done the same thing in your situation. I hope your able to get him into the vet first thing tomorrow. Just remember not to give Rimadyl with the steroid.

Hi Dot! Welcome once again to Dodgerslist!
Sorry to hear that Dudley is having another disc episode. Glad you crated him right away.. It was ok to start him on the Tramadol because it is short acting and at least he will have a comfortable night and that is a good thing.. The Prednisone, however isn't such a good idea because it limits the Vet's choices in what to treat him with.. Once started they have to be continued OR a 4-7 day washout to change to another type of anti inflammatory. Also it may cause his symptoms to be less obvious and make it harder for the Vet to determine what is going on.. Great job on starting the Pepcid!
The important thing is he is crated and comfortable tonight and tomorrow let us know what the Vet says about his current IVDD episode..
Sending prayers for you and Dudley. I'll be thinking of both of you..

Thanks for the quick reply!! Got him crated and next to my bed on an ottoman. Hopefully I can get the Vet on the phone in the morning and see what they say about his prednsione and whether he should just stay on them at this point since he did well on them the last time. I only gave him 1 dose. For now he still has bladder control, wags his tail, can stand but doesnt want to move much. Doesn't seem to be any knuckling so those are good signs. I'm just glad I was paying diligent attention and hopefully caught it early enough! Thank God I had the dodgerslist flyer on my fridge to confirm the signs!

I called Dudley's Neurologist this morning and got an appointment for him Monday morning at 11:30 am. She gave me the ok to administer his Tramadol, Pepcid and Prednsione . Prednisone 5 mg ... 1/2 tablet every 12 hours, Pepcid AC 10 mg at 1/4 tablet once daily and Tramadol 50mg 1/4 tablet every 6 -8 hours through weekend along with his crate rest. I work tonight from 3-10pm so I've already made him charts for his meds and have it timed for his last dose for today when I arrive home from work. Prayers are very welcomed and appreciated. We hate seeing him go through this again I'm just so thankful that this time around he's not dragging his back legs and can still stand. He's squatting to pee and hasn't popped yet today but I will try to get him to go again in a few.

Dot, so sorry to hear Dudley is having another disc problem. Your crating him at once has likely prevented diminishment of neuro functions. You might want to add 1 teaspoon of pureed pumpkin plus for each kibble meal soak it in equal parts of water or broth to help with constipation that Tramadol can cause. Note high fiber alternatives: really ripe mashed fresh pear, just take off the peel off; microwaved and mashed peeled sweet potoato. I think that is very smart to make a med chart to keep all the meds straight and avoid skipping doses. For others following Dudley you can find a sample med chart here: www.dodgerslist.com/literature/crateRRP/medchart.pdfDo keep us posted on the neuro visit on Monday morning. We are always looking for vet recommendations to help new members…vets who know IVDD and conservative treatment. Check out our directory here if you can add to it we would appreciate it: dodgerslist.boards.net/board/10/guidelines-posting

Thanks Paula... I added his vets last year to that link:) I'm going to pick him up some chicken breast, sweet potatoes and some cans of Wellness. I do remember last time it messed up his appetite and he would refuse to eat at times when he needed food with his meds. I will be prepared this time!! I also remember the thirst and high volume pees and tons of washing his bedding. I guess this is the only good thing of his relapse... Almost made a full year but I remember everything like yesterday!

Our Forum is new since March of this year, so I hope you will be able to again (sorry about that ) add your vet recommendation to our new directory: dodgerslist.boards.net/board/10/guidelines-postingDot, this is what we hope all members do is to read, remember and be able to quickly apply what they have learned should another disc problem occur. Good job Dot!!!! While he is on pred, you likely would need to take him out to pee every 3-4 hours due to the extra water he is consuming.

ok Paula...I added his specialist info. I do remember to take him out every couple hours which is not a problem for us...we have 3all together and have always let them out every 2 -4hours when we are home anyways. We have a fenced in yard and find that letting them out often prevents accidents in the house....we have been carrying Dudley to his potty spot then afterwards we pick him up and place him back in his crate.Last year the longest Dudley could go while on the Pred was 2 hours max or I was changing out his bedding. Still no poop yet today...I don't think its because hes constipated at this point but because his meds haven't kicked in yet. I have faith he will poop eventually so Im not going to panic yet. I may boil him some soft carrots since its all I have right now to help soften his poop

Good news...Dudley still has bladder control just took him out and he sniffed and peed...still no poop though..i think hes just scared to try right now....I know it will eventually come out sometime. on another note... panting has started from his meds...im also familiar with this side effect so Im not going to panic like last time...this too shall pass. Were all just trying to remain calm and relaxed so he can rest and his body can heal. Makes me sick to my stomach that I have to leave for work soon and leave him home alone but I know he will be fine in his crate...deep breaths...think Im going to leave some soft music for him and give him his Tramadol before I leave for work. Im going to expect to change out his bedding when I get back and I am certain any accidents aren't because of no bladder control but because no one would've been home to let him out and his medication will cause him to go more often. HooRay!! Dudley pooped right before I left for work!!

good news on the poop! For the panting you can place a fan near his crate but not pointed at him…to give some air circulation and make him more comfortable.
If no one can come home to give him potty breaks, can you put a pee pad in part of his crate so he does not have to sit in wetness?

I had a water proof type pad and some absorbent fleece in there while I was at work. He did pee in there but it was expected. He has always peed in his crate since he was a puppy even though he's house trained. He's 10 yrs old and set in his ways. I do know that he has bladder control since he sniffed and peed outside today several times. I also left ceiling fan on for him... It's very hot here in Fl.
My hubby just got home from work and took Dudley out potty this time... Dudley did pee and poop!! Never thought we'd be so happy over poop!!

Having a good morning with Dudley so far. Hubby took him out potty again and got pee and poop so we are doing well in that dept. I find that his appetite is getting weird again... Doesn't want his kibble... He wants the good stuff like steak and salmon! Lol! I've had to outsmart him some here... I take his kibble and soak it in hot water, add some veggies and a little chicken broth and blend in my Ninja. He's been eating it so I guess he likes it! I wanted to make sure he's getting extra fluids and his diet is complete this way. Maybe this idea can help someone else? Going to get some pumpkin today to add to it too.

Quick question.... Is it really nessessary for Dudley to get an X-ray tommorow if the vet suggests to? We already know he's having a disc episode and we don't plan on having surgery due to finances. In our opinion an X-ray would be more money to confirm what we already know. We can't afford to pay for an X-ray....thanks for any thoughts...

Ask your Vet if anything found on a radiograph would change your treatment plan. If the answer is no, then skip the xray, and you can put your money into other treatments like accupuncture or laser therapy if it is in your budget. Sometimes a radiograph will confirm IVDD, sometimes not. If the treatment will be the same regardless of what may be seen on xray you can save your money - the xray would only be to satisfy curiosity. ("curiosity" doesn't sound right, but you get what I mean ) Keeping you in thought and prayer

Thanks Stevie for the affirmation ! My thoughts exactly. I just hate it when you go to the Vet and think they have the best interest in your pet but really it's all about how much money they can try to squeeze out of you. The specialist we go to is awesome however at times it feels like high pressure sales pitches for surgery. We do feel it would be a waste of money on an X-ray especially if the treatment result will be the same (conservative) a good vet should be able to confirm IVDD just by examination alone... I really appreciate you confirming my logic! ~ Dot

Hi Dot, I agree with Maureen. If your vet (is this a specialist ?) can't give you a very good reason or why he might suspect something other then IVDD, I would skip that x-ray.
Yes, specialists like to do surgery. That is what they do and most do consider IVDD a surgical disease. Not all do, however, and we are finding more and more who will treat conservatively. Sometimes you have to be very direct and let them know that is what you want to do and ask them to help you do so. Most will but there are some still out there who still just want to do surgery. Like everything else, sometimes we have to look around to get what we want.

Thanks Linda and yes this is a specialist. I learned a lot from his first go around with last years IVDD episode. The specialist is the only one in my area that is closest to us that we are going to. Same one we used last year. I'm just preparing myself for their sales tactics in advance so we don't end up paying more than we really need to. Last year they were on board with the conservative route. Ill update tommorow after we get back. I was told to be there at 11:30am but the Dr is booked solid so we may be there awhile waiting since they squeezed us in. I understand that they all have a passion and heart to treat ill animals and I also know part of their job is also to bring in more money. Tomorrow I will be Dudley's and our finances voice!

Just got back from Dudley's neuro appointment. The Dr. Was able to confirm he is having a mild disk herniation without X-rays of course as expected they tried to sell us more X-rays and surgery option again but we declined. They weren't able to tell if its the same disc or a new one so they are guessing its T3 -L3. Treatment is the same regardless. They did mention since his last visit he's lost about 1lb......I told them that that's normal for him in the 10 yrs we've had him... He always fluctuates between 7 and 8 lbs... He's weighing around 7lbs as of this morning. Since he lost some weight they went ahead and lowered his prednisone to 2.5 mg 1/2 tablet every 12 hours for 7 days then 1/2 tablet every 24 hours for 7 days, 1/2 tablet every other day for 14 days then STOP Tramadol was adjusted to every 6 hours instead of 8hours @ 50 mg 1/4 tablet Pepcid AC 10 mg 1/4 tablet once daily They also mentioned when they did his exam he didn't yelp when they pressed on his back however it is still arched and he did make slight noises that he was uncomfortable with handling him in general. They also ordered only 4- 6 weeks crate rest ( but were still going to do the full 8 weeks because we know better!) he's been pooping and peeing every time we take him out so that's also a plus! Thank goodness we caught his relapse early and we retained all of our knowledge from the last time!!overall visit went as well as expected!! We go back for a recheck in 2 weeks...

Dot, the taper of pred is the time to be particularly observant for any signs of pain coming back which would indicate that not all of the swelling is gone in the spinal cord. Typically the pain meds would be backed of or stopped so that a clear picture would present itself about pain. Your vet has instead increased pain masking Tramadol??

Post by Dot & Dudley on Jun 17, 2013 14:07:28 GMT -7
I don't understand your question Paula.... I'm aware to keep a close eye on him during his taper. She adjusted the amount of Pred because he lost 1 lb. and according to the weight chart .she didn't really increase the frequency of the Tramadol... It was to be given every 6- 8 hours originally but told me to go ahead and just do every 6 hours since his herniation is fresh.

I think that there was concern that pred may start being tapered in 7 days while still on Tramadol every 6 hours. Usually the pain medication is reduced or stopped prior to the tapering of the prednisone to once a day, since a return of pain usually means the the prednisone needs to be continued for a longer period at twice a day to combat swelling. I hope that his helps :)By the way... you are an awesome advocate for your little guy - way to go!!

Gotcha yes I remember to taper on the tramadol too the same time as the Pred taper. I think the confusion was the way the question was asked..... Was difficult to comprehend. Thanks

He's not on his taper yet... She wants the taper started in 7 days from today. So that would be Monday June 24th. We shall just have to wait and see how he responds until then. Fingers crossed he will go just fine.

Can someone refresh my memory.... How long does the arched back stick around?? Dudley's back is still arched/ hunched outside on potty breaks and I can't remember how long he had it last time. Does this mean his inflammation/ pain is not resolved? He doesn't seem to be in pain however I think his hunched back means he still does ... Thanks

For the record Stevie has adopted a slightly hunched stance to compensate for her weaker hind end - looks odd, but not caused by pain - believe me we had that checked out thoroughly

I know he's compensating for comfort reasons... Just seems like last year it eventually flattened back to normal.... I just can't remember how long it took. I'm guessing it varies from dog to dog and to be patient.
