STRICT means: - no laps - no couches - no baths - no sleeping with you - no dragging or meandering at potty times. - no PT - no chiro therapy Why Chiropractic is not recommended for pain for an IVDD dog: dodgerslist.com/2020/04/22/chiropractic/
One vet did recommend Chiro next week. we will not do Chiro and review the link above.
☆ 7 bladder control? yes
DOGs with BLADDER Carry to and from the recovery suite to the potty place and then allow a very few limited footsteps. Using a sling (long winter scarf, ace bandage, belt) will save your back and help to keep a wobbly dog’s back aligned and butt from tipping over. Will start to use scarf for now A harness and 6 foot leash is to control speed and keep footsteps to minimm as you stand in one spot. An ex-pen in the grass is an excellent alternative to minimizing footsteps with the physical and visual to indicate there will be no sniff festing going on! we will set this up
☆ 8 wobbly walk? Yes

Welcome, Uecker! We are glad you have joined us all. Pain is an important symptom to make sure it's being correctly identified. Better pain is not good enough. --- Pain during the course of pred prior to 9/27 test taper day, means the vet needs to be alerted. Expect another course of pred, adjustment in pain meds and is it not time to taper. A 3-day pred course is a short one. Often vets will try a 7-day or even a 14-day course. So let us know for sure if you are seeing any signs of pain when having to move, at potty time, nearing the next dose of the single pain med gabapentin. IF pain, indeed, then... Expect: -- Gabapentin for nerve pain to be 3x/day as pain meds last for 8 hrs. -- adding in methocarbamol for muscle contraction pain 3x/day -- traMADol as the general analgesic 3x/day Let us know specifically if you ARE seeing pain and what the vet does to adjust meds. STOMACH PROTECTION Pepcid AC (famotidine) lasts for 12 hrs. Geoff's 16 lbs should be on 5mgs TWICE a day (every 12 hrs)How is his stool now after being on lactulose regarding constipation? Now back to a normal Play Doh type texture?
Which type of recovery suite do you have? Water/food bowl attached inside the suite? Wire crate, ex-pen, child's Pack N Play. This link explains the detail necessary to provide strict limited movement of the back in order for the disc to heal. Also tips in setting up the suite, it size, and other things to make the next 8 weeks of STRICT rest go smoother. dodgerslist.com/2020/05/14/strict-rest-recovery-process/

Hi Paula, thank you so much for the quick response! Your time and information is greatly appreciated.
The uncomfort I have been seeing is when I pick him up and he does a small moan. his tummy is large from the prednisone but I now realize the vet did not show us how to pick him up correctly. He showed us a way to pick him up where his body is up/down vs side to side. So I will be adjusting this and see how he does next time I pick him up to potty. The other time is when he does potty, we just started the stool softener and his poop is hard yet. When they did his enema they said his poop is hard as a rock. I did buy some [name of?] digestive enzymes I am hoping this helps also to move things along in his system. I did call the vets and brought this up this morning, along with a few other things and am waiting a call back. The vets believe his pain when he goes poop is from the poop being so hard, hopefully the stool softener will kick in soon and improve that.
I will ask about the prednisone and see if we can do more then the 3 days at 2x a day dosage. usually the 2x a day dosage is at least 7 days before they taper off? and get more of the gabapentin too. He does not have pain from moving, he did try and move his blanket around to "make his bed" and I had to make him stop bc he was moving so much.
I will add in another dose of the Pepcid.
Thank you so much, so far I am using a crate. His food/water is outside the crate and I am using a tray to put his dishes on so he is not bending so low. when i am up, he is resting on a memory foam dog bed surrounded by pillows so he doesn't go off the bed. I am going to do a different set up and am trying to buy things to do so like the one above w/ the suite.
I was looking at a gingerlead for small pets to help his back when we go out to potty. unless you have another suggestion on which kind is better for his breed?
Is there a time when to start laser therapy or acupuncture type services? I was possibly looking into that and see what we have in our area.
thank you so much for all your help, and suggestions and advice. We are greatly appreciative!!

Uecker, yikes this is not good at all that a vet did not have the knowledge on correct way to pick up an IVDD dog! I hope the picking up like a baby has not impacted this disc trying to heal and form strong scar tissue. Your dog is in pain because of having to move too much to poop. Disc episode are very painful. A normal dog even with constipation would not be in pain but just have to exert more to get the poop out. Sounds like clearly a 3-day pred course is not likely to be long enough to even consider a taper on 9/27. That constipation needs to get corrected soon! Vertebrae movement to poop could be damaging the early healing disc. Too much pressure could cause the disc to rupture than now which is a suspect of a buldge. The usual is a 7-day or a 14-day pred course, then begins the kinda of a test tapering to find out if pred had done its job. Pain meds are also naturally stopped that same day because they block YOUR ability at home to accurately and promptly assess for pain. Let us know you and your vet's Plan B for the taper: With any TAPER, it is always good to have worked out in advance a "PLAN B" should pain re-surface at night or on the weekend when your vet is not open. Could be an emergency RX script you could fill at local 24 hr pharmacy or some extra pills at home til the vet opens again. An ER visit is very expensive, a "Plan B" is free! RULE OF THUMB Pain= another course of PRED + all pain meds, GI protectors back on board. No Pain= go to TAPER conclusion...finish out the 8 weeks of crate rest for the disc to heal.
Food and water bowls should be attached at head height inside of the suite Gingerlead is a fine belly sling to use with your own harness. A long winter scarf, a belt or ACE bandage can work also in conjunction with your own harness for potty times. The single most important care is the at home in the suite STRICT rest of limited movement and limited footsteps. If you must have laser or acupuncture, hire a mobile vet who comes to your home. The focus right now is getting the disc to heal. Nerves may or may not heal in the short 8 weeks a disc does. The body will self repair nerve damage. Think more in terms of months rather than days weeks. for this slowest part of the body to heal....nerves. Can you be more specific on what you observe about his wobbly walk. --- do his back paws knuckle under? Can he correct them slowly or not at all? --- do his back paws graze the ground. --- or do his back legs sometimes cross.

I know I am very scared about all this. the vet tech called back and did -- extend the ▲prednisone to 5mg 2x a day for 5 days, then 1x a day for 5 days, and then 1 a day every other day until gone. [MED LIST/HISTORY- Moderator's Note. Please do not edit 16lbs 10-11y.o 9/23 wobbly, right rear paw knuckling Dex inj 9/23 prednisone as of 9/23: 5mgs 2x/day for ▲8 days, 10/1 test taper for: _pain / _neuro gabapentin 100mgs 2x/day thyro-tabs 2x/day lactulose 2x/day Nusentia's Enzyme Miracle Pepcid AC 5mg 2x/day ] They also will refill more of the gabapentin with 30 days worth and its 100mg 2x a day as I did bring up the backup plan and what if something happens on the weekend. I have to contact them back and let them know how things are going as we go along. I mentioned he seems to be a little "puffy" and they said Prednisone doesn't cause that. Hm everything I read says they could have a pot belly appearance? They did say Milk thistle and Pepcid are good to add in. yes I hope the stool softener kicks in soon! very worried about that. He said if he ends up w/ diarrhea then to stop the stool softener, he was holding in his poop before but this morning 9/25 he went right away once we went outside. Its not playdoh texture yet but its not hard like a rock either. would the digestive enzymes help? they are called enzyme miracle by by nusentia. I tried the scarf and he froze ,and wouldn't do anything. HIs legs initially were very wobbly, legs crossing, one paw did the knuckling, the other did not. He was able to correct the one paw right away. The other paw he said he was 9/23]trying to correct but couldn't on Saturday during the exam. [9/25]:He walks a little wobbly, but he is more straight today and both back paws on the ground. his back doesn't look as hunched like yesterday. If he wants to stay laying to eat, is that an issue at all? I need to buy different dishes. I will be looking on amazon and buying as much as I can to set up the correct suite. Thank you so much for all your help, its greatly appreciated!! The specialist vets are 2 plus hours away, we are limited w/ vetting in my area and really appreciate the information and help.

Uecker, good job on advocating for extension on pred. Kudos to you! Excellent to hear that getting pred on board has so quickly turned neuro diminishment. That is the power of pred if it can be in use quickly. So to confirm: 9/25 not as wobbly " the rear paw (left or right?) no longer knuckles under " a less painful hunched back PREDNISONE and GABAPENTIN Is this now the prednisone anti-inflammatory level dose prior to the Oct 1 taper starting? On Oct 1, will gabapentin be stopped?Since you've been reporting pain what is the reason they did not increase Gabapentin to every 8 hrs (3x/day)? Gabapentin is effective for 8 hrs.PUFFY- DIARRHEA Details are important because we can't see Goeff. Is the puffy in the stomach. That could be from too much poop still remaiing in the digestive tube? So glad to hear texture of poop is greatly improved. As vet says key an eye onb poop to stop lactulose in time before you get diarrhea.ENZYME - Milk Thistle Thank you for sharing full info on what he takes. Is Milk Thisthle on board? Not sure the digestive enzyme would help with constipation, but can't harm either. While on pred he should be drinking more, a benefit to to help with constipation (better hydration). Had there been a raw food change to account for constipation. What does he eat, brand?
EATING No problem if he wants to lie down to eat. But that sounds like a sign of pain: reluctant to raise up to stand. We can see Geoff, so can you paint a picture for us: Is he lying down on his side? Or does he lie on his belly? Many male dogs just do not like anything touching their belly. Especially at potty times. So he may not do well with a GingerLead. Try the DIY figure 8 sling for Goeff's rear wobbly legs. Keep up the good work in monitoring Geoff and you dedication to ensuring a good healing environment.

it was the right paw knuckling [9/23] on Saturday.
I am not sure but I will check. I sent his records to our other vet for review to and see what they say. they wont send the prescription to another or pharmacy, its $20 for 10 capsules on the gabapentin. Trying to do all these meds every 12 hours. we are up at midnight doing meds LOL.
He pooped 3x yesterday, we will see how today goes. I still have the stool softener on board.
I was reading on milk thistle, I think I have to wait until after the prednisone to add it in? because it says it could increase or decrease the affect.
He hasn't been drinking much even with the prednisone. I have been adding in water with his canned dog food, he has been on prescription GI Biome dog food due to tummy issues and anal gland issues. We have tried many other dog foods in the past with no success. He can't go long w/ an empty stomach so I have to split meal up 4-5x a day. Hopefully the enzymes will help.
He has been getting up to stand and eat.
he lays on both,, sometimes belly sometimes side
I will try both, I already purchased the gingerlead. He is a very touchy boy, He was a rescue out of state, living on the streets and ended up in the shelter 3x and on the kill list. I am not sure what he will tolerate.
Thank you so much for your help!! I ordered more on amazon to help with his healing - crate bowls etc.

Uecker, warms my heart to hear how diligent you are in checking out meds such as Milk Thistle before giving with pred on board. Providing us with all details helps us to see the full picture about Geoff treatment. So thank you for naming the food he eats, which paw knuckled but now does not, going poop more often and standing to eat, etc. Hopefully the Pepcid AC alone will be doing an excellent job of stomach protection. The Hill’s Prescription Diet Gastrointestinal Biome is a highly processed high-fiber food. Not a raw diet in any way. You giving it on top of an actual raw diet? Your vet is selling it to you? Ingredients listing Note: #5 and #6 ingredients are RICE!!!!! Read on below about relationship to your reported "puffy" and what rice does to dogs. This Hill's very high fiber wet food...their quote: "is shown to help FIRM loose stool in 24 hrs" Jeesh, no wonder poor Geoff is suffering with constipation! And then has to be on a strong laxative to reverse what the Biome did, what the heck?*! HIGH FIBER Just for your own information on high fiber foods and how they work and what Dodgerslist recommends for these situations: Plain pureed canned pumpkin is a magical fruit - its high fiber can firm up stools and help with diarrhea or loosen the stool to help with constipation. Pumpkin (unlike commercial preparations i.e. Lactulose) is an actual food with minerals, vitamins, etc. NOTE: alternatives are really ripe mashed fresh pear, just take off the peel off; microwaved and mashed peeled sweet potatoe -- To loosen the stool, add equal parts water to each kibble meal and soak overnight. At mealtime add 1 to 1.5 teaspoons of plain canned pureed pumpkin 1x a day. Another way to hydrate are some frozen broth ice cubes to lick on during the day since you feed a wet food. -- To firm up the stool add 1 to 1.5 teaspoon plain canned pureed pumpkin 1x a day to kibble. No extra water. Give a teaspoon of pumpkin for every 10 pounds of body weight, NOTE: Rice is complex carbohydrate that tends to ferment in the dog's colon, causing unfortable gas. In addition rice often passes right through the dog's GI tract exiting totally undigested. Whereas pumpkin or sweet potato is nutrient rich plus high in fiber. Dogs can absorb/digest its nutrients from the fiber. Figure 8 Sling Hopefully Geoff will accept the figure 8 sling at potty time. Otherwise the other way is a bit of a back killer for you. You would bend down to support his wobbly back legs by supporting his thighs with your hands. GABAPENTIN $2 per 100mg capsule(?) is really an outrageous price. Prices vary pharmacy to pharmacy and day to day prices can change, so you gotta call and shop around. Also there is a break point where the price/pill(or capsule) drops lower. So ask pharmacist where price point drops. Then explain to your vet why you want... say a 30-day 3x/day Rx to gabapentin to a particular pharmacy. Then you have plenty on hand if Rx'd 3x/day. Not fair for Goeff to be denied full relief from pain because the meds are not being adjusted. Gabapentin lasts for 8 hrs. Also note by law you do not have to be a member of Costco or other Big box stores (if you have one nearby) to use their pharmacy. So include Coscto, Walmart and other local pharmacies in your price checking. What is your city/state? SLEEP As the care giver it is important you are getting a good night's sleep. Try moving back or up his pills by an hour ea day till you achieve giving last dose of the night at a reasonable 10pm or 11 pm schedule??? A medication chart is useful to see patterns, have dates and specific facts handy as you discuss things with the vet. D/l a sample and a blank form to use with your dog's meds dodgerslist.com/wp-content/uploads/2020/02/med-chart-printable.pdf

Thank you so much for all this information, I will ask the vet about the meds and food today when we go there to pick them up. I know the prices seem outrageous, esp on the gabapentin when I was looking at the pharmacy's. We wont have any left, so I will unfortunately have to pick them up today from them. I will ask again about a script to a pharmacy to see what they say, a lot of vets around here do not allow it. We live in Central Wisconsin. I hear Wisconsin is one of the areas with expensive vetting costs. Hopefully they have another food recommendation for stomach/anal gland issues due to his hard poop. I was giving pumpkin before this all happened, and thought maybe the pumpkin was causing the constipation. I always give fresh foods, veggies a lot, along w/ his food and pumpkin happened to what I was using before all this happened.
Geoff does seem to be doing a little better today. I can tell he wants to be out more, but nope, he is denied LOL. He did take another poop today [9/26], same texture as yesterday - but he didn't struggle as much like he did previous days. Good sign there!
I hope I am able to do something for his support. We will try all the above. He doesn't like his legs touched, even when I put on a regular harness. So we will see how things go, and try anything to help him.
Thank you. I have been wanting to space everything out for him due to tummy issues and did not want to give everything all at the same time as the vet suggested. The nightly appointment Friday was when his meds started up so it was kinda a awkward w/ timing to.
Thank you so much for all your time and help, its greatly appreciated!!

Uecker, like hearing he's not struggling so much to poop. Means less pressure on his early healing disc... THAT IS A GOOD THING!
Glad to hear Geoff is feeling a little better. Still a "little better" is not good enough. What specifically do you observe about pain: --- nearing next 12 hrs dose of gabapentin --- when he has to move such as at potty time --- other?
So the diet has never been 100% raw food? Then before this disc episode what was the main food brand name (wet or dry?) that you enhanced with raw veggies and what specifc kinds of "fresh" foods, meat, etc.?

Yes, the stool softener must be helping now. Yay!! He took a very big poop earlier today and this afternoon/evening has been a lot better. The texture was still the same. I have not seen any pain from him now. His personality is coming back, he was wagging his tail more, tried to give kisses, bathing himself like he does. He doesn't hesitate as long to go to the bathroom- we went out to potty and a couple steps he went pee right away.
No, we have tried so many foods, different brands, different kinds and he can't handle much besides the canned dog foods. So I enhance his canned food with lettuce, he loves "salads" he likes romaine lettuce best out of the different kinds. any type of veggies we have examples could be cucumbers, green beans, pumpkin, asparagus. meats are depending what I have to, chicken, turkey, beef. I add in scrambled eggs in the morning. sometimes low-fat cottage cheese, or yogurt to. I can try another canned dog food and see how things go. we have tried so many in the past bc of his stomach issues and anal gland issues, I think living on the streets messed up his tummy/anal glands.

Uecker, what a good report on bowels getting back to normal and making Geoff feel like himself again (out of pain). Thank you for the detail so we can visualize what you observe!
Now would not be the time to change up his food to a different canned food. If there were to be loose stool you wont know if due to merely temporary thing caused by change in food vs. a serious thing of stomach damage. Same deal with the lactulose and why to stop it before there would be loose stools. Keep an eye on things in mind for stopping lactulose.

Ok thank you, I will not change his food at this time.
I am wondering when we go to one prednisone a day vs the 2, am I able to switch the time given?
With the weird schedule bc of the vet appointment timing etc. We now do, 8am/8pm prednisone and 1030am/1030pm gabapentin. However bc of his thyroid meds, the Pepcid should be 3-4 hours after the Thyro-tabs. So if I do the last prednisone at 8pm the night before...
and the next morning switch Gabapentin w/ breakfast 8am/8pm... then do Pepcid at 11am w/ prednisone at 1130am w/ his afternoon meal?
Thank you so much!! Your time and help is greatly appreciated!

Uecker, did not see Thyro-tabs usual dosing time, hopefully below will still be of some help in your new schedule. 9/28 7:30a Pepcid AC (2x/day) 8a prednisone (1x/day) gabapentin (2x/day) + breakfast 11:30a Thro-Tab (2x/day) (4 hr separation from Pepcid) Nusentia's Enzyme Miracle "FEEDING GUIDE: Add 1 scoop (approx 1/2 tsp) for every cup of pet food at each meal"PREDNISONE dosing by pill (Exogenous) The maximal activity of the adrenal cortex is between 2 am and 8 am, and it is minimal between 4 pm and midnight. Exogenous corticosteroids suppress adrenocortical activity the least, when given at the time of maximal activity (am). Therefore, it is recommended that Prednisone be administered in the morning prior to 9 am. www.drugs.com/pro/prednisone.html

Sorry about that. The thyroid meds have been a unusual schedule bc of my schedule. He gets the morning dose at 7am, I wasn't sure how to keep the thyroid meds and do the Pepcid in the mornings?
I wasn't sure if I could add in the digestive enzymes in w/ everything or wait? I was hoping those would help keep things moving thru his system. He hasn't gone number 2 [9/28] today.
Thank you so much!!

According to the Nusentia's Enzyme Miracle "FEEDING GUIDE: Add 1 scoop (approx 1/2 tsp) for every cup of pet food at each meal"
See if this schedule works or can be modified to suite your situation.
9/28 7:30a Pepcid AC (2x/day) 30 mins before pred
8a prednisone (1x/day) gabapentin (2x/day) + breakfast w/Enzyme Miracle® (giving a meal with pred could help with stomach protection.)
11:30a Thro-Tab (2x/day) (4 hr separation from Pepcid)
Before all the constipation, what was Geoff's normal number of times/day to poop? My dog pooped 1x/day. Other dogs have different schedules. Maybe Geoff's body is getting back to his normal schedule? ? Let us know what you observe today whenever he poops re: if approaching a normal texture.

Thank you so much.
Oh ok, I wasn't sure if I could switch his thyroid med timing due to him being on the same time for years. His system will hopefully adjust fine temporarily to the while on the other meds. I also forgot about the stool softener times, I've been doing that now at noon / midnight w/ food. augh. So idk if that would work w/ the thyroid meds. I may have to move the schedule up to earlier in the morning.
He did have another big poop last night after I responded on here. The first couple were still the same texture as previous, but the ones after were softer. He used to go 3-4x a day on our walks. I would be sure I walked him every day if not twice a day bc he wouldn't go unless we walked. Idk if we need to ask the vet for more of the stool softener to be sure he goes, bc now w/ him not walking I am worried he will back up even more then before. Hopefully the digestive enzymes help move everything thru better though.
Thank you so much for your help!! This sure is complicated w/ the schedule and timing of meds.

Uecker, normally med times can be changed. But it is done gradually by moving time forward or backwards incrementally by an hour as mentione in an earlier post. Best you run your proposed schedule by the vet to make sure of the things you don't know such as lactulose with Thyro Tab, etc.

Thank you so much, I will discuss w/ the vet. I really appreciate all your time and help!!
Hi Paula, checking to see if you are on?
We tried the 1x a day prednisone [9/29 TEST TAPER] yesterday as advised from the vets (5 days 2x a day, then go down to 1x a day). This morning is not going as well now [specific signs/observations: pain/neuro?] This [9/30]morning he was shaking while standing and did not want to use the restroom. and more wobbly again w/ walking.
Yesterday [9/29] he was doing so well, he was standing taller, not wobbly, no pain etc.
- he was improving and doing so well yesterday to - now the vet wants to do the incorrect info: full 14 days at 2x a day, then go down to 1x a day for 7 days and then every other day. [MED LIST/HISTORY- Moderator's Note. Please do not edit 16lbs 10-11y.o 9/23 wobbly, right rear paw knuckling Dex inj 9/23 prednisone as of 9/24: 5mgs 2x/day for 5 days, 9/29 test taper for: 9/30 pain / 9/30▲wobbly neuro as of 9/30: 5mgs 2x/day for 7 days, Sat 10/7 test taper for: _ pain _ neuro gabapentin 100mgs 2x/day thyro-tabs 2x/day lactulose 2x/day ]
Only thing, the pepcid interferes with the lactulose. So we cannot do the pepcid. He said to keep him on his same thyroid medication schedule.

Uecker, sorry to keep asking things. Since we are not there we depend on your ability to paint us a picture (using dates, all details about a med (mgs, x/day, etc.) and the "answer" if you had asked "why, what is the thinking behind that?" 1) What were the pain or neuro observations that explain what "not so well" means?
2) What are all the details of pred change? as of 930 pred dose in mgs? 2x/day for 5 days; 10/14 test taper for _ pain _ neuro It is not unusual to see owners reporting vets Rxing lactulose but not calling for a stop of Pepcid AC. So this was quite a surprise to me. But then Geoff is on a Thyroid med that is not in are area of knowledge. You have reported while on lactulose AND taking Pepcid AC (famotidine), the stool was progressing to a better texture. Hmm that appears to show may there has not been an interaction for Geoff???? Web MD: "Lactulose is a colonic acidifier" Drug interaction with famotidine: listed as "0" You can do a search your self on "Lactulose and famotidine drug interaction?" to further learn more. The concern always with use of any anti-inflammatory is the protection of the GI tract from potential of deadly bleeding ulcers caused by too much acid from lactulose, prednisone, change in routine stress and spinal cord damage. Of course, don't want a dog to be constipated either. Surely there is a happy medium approach? Did you ask your vet, then about plain pureed canned pumpkin instead of lactulose with Pepcid AC? other options? --To loosen the stool, mix into a meal of canned wet food 1 1/2 teaspoon of plain canned once a day. Make sure this extra fiber is being accompanied by extra intake of fluids. Freeze low/no sodium meat broth into cubes to lick on during the day. NOTE: alternatives are really ripe mashed fresh pear, just take off the peel off; microwaved and mashed peeled sweet potatoe

Please let us know now after adding another course of pred, has the wobbly kinda turned around (less wobbly now?
Are all the signs of pain now under full control with another course of pred? Have no patience at all with any hint of pain. No reason for pain when pain meds can be properly adjusted.
If pain not in full control then: Why was gabapentin not adjusted to give promptly every 8hrs? Why was methocarbamol not added to ease the pain of muscle contraction 3x/day Ditto for: traMADol which is the overall analgesic 3x/day
Was the pred course adjusted over the phone? Or did you do a car transport into vet's office?

It was adjusted over the phone. They mentioned to either keep him on the 2x a day prednisone a little longer, or do the 1x a day prednisone with the methocarbamol added in. I decided to go back to the prednisone 2x a day because he was having a lot of improvement yesterday / no pain sings, and the above notes mention the prednisone should be the 7-14 day course, and the vets did 5 days so far 2x a day. So hopefully this is the correct thing to do, and his pain goes back to zero like it was before and we see improvements again.
Thank you so much for the reply!
Yes yesterday we made such a big improvement. this morning however,he is not feeling well - as in - [9/30]shaking again during potty breaks, and wobbly [increase or same as 9/29?] walking.
Sorry, I talked to the vet tech [9/30] this morning and she gave me incorrect info. The prednisone is now 5mg 2x a day for 7 days, [TAPER DOSES BEGIN on 9/6]: 1x a day for 7 days, then every other day. vs the 5mg 2x a day for 5 days, 1x a day for 5 days and every other day. Since we did the prescription 5 days so far at 2x a day - and he added another 7 - that would give 12 days of 2x a day. confusing!
I did have to search more on the Lactulose. Antacids can interfere with the colon acidification process and make lactulose less effective in trapping ammonia. That could be why its taking so long for him to have the normal softer poop. Its not hard as a rock like it was, and is softer .... but its still not soft soft like playdoh. I did not do the [9/29 STOPPED] pepcid yesterday and he ended up having 2 poops, and getting more soft than it was previous days. This is complicated bc of the additional issues w/ constipation along w/ the IVDD. The research mentioned we can't do tums even or anything w/ antacids w/ the lactulose. I will have to ask the pharmacist when the refills come in on Monday and see what he says.
I am not sure if I can add in the Enzyme Miracle bc of any interactions? I know the Milk Thistle had interactions w/ the prednisone so I have to wait on that one. Anything else for an alternative for the pepcid for now? I had my other vet review what this vet is doing for treatment etc and she agrees on everything this vet has been doing so far.
He was doing so well yesterday - no signs of pain, standing up straight, not hunched, etc, I will keep you posted on the next couple days. Hopefully we will turn around again, going back to the prednisone 2x a day.

Uecker, so very glad Geoff did NOT get 14-day course of pred, but rather a 7-day course.
-- 5mgs Pred twice a day is the anti-inflammatory level dose needed to work on swelling -- The pred course length is what we are interested in. It can take 7-30 days on the anti-inflammatory level to get all swelling down. Taper days serve a different purpose and are too low to count in the 7-30 days. -- all pred doses after the end of the course are taper days as they are less then 5mgs 2x/day.
You are right this is complicated stuff with Goeff's other issues. I'm gonna ask you to help us. Get out your calendar and use a date (do not use yesterday, tuesday, last week or general ideas like that). We always re-ead your posts each time before replying. Every time we have to spend time recalculating what general references to days are.
To tell you the truth I'm lost with the prednisone thing. Keeping track of how many days up at the anti-inflammatory level (5mgs 2x/day) is important for you as well us trying to make good comments. Is the concise/brief list (sequence of events) below correct? If not fix the dates for us, please in your next post
MED LIST/HISTORY 16lbs 10-11y.o 9/23 wobbly, right rear paw knuckling Dex inj 9/23 prednisone as of 9/23: 5mgs 2x/day for 5 days, 9/28 test taper for: 9/30 pain / 9/30▲wobbly neuro as of 9/30: 5mgs 2x/day for 7 days, Sat 9/7 test taper for: _ pain _ neuro gabapentin 100mgs 2x/day thyro-tabs 2x/day lactulose 2x/day issue with stomach protectors due to Thyro tabs and lactulaose
Today, Sat 9/30, is there pain still? If yes, why has not methocarbamol been added and gabapentin moved up to 3x/day? These two pain meds are effective for 8 hrs, thus the need for 3x/day dosing.

Thank you. Yes I am confused myself w/ the prednisone. Sounds like we are doing the prednisone correctly? 12 days total of 5mg 2x a day which falls into the 7-14 days. We started 9-23 w/ one prednisone at night after the vets appointment. 9-24 started the 2x a day prednisone, 9-24 to 9-28 = 5 days of the 2x a day prednisone. 9-29 [test taper] went down to 1x a day prednisone 9-30 go back to 2x a day prednisone for 7 days, 9-30 to 10-6 10-7 [test taper] go down to 1x a day prednisone for 7 days 10-13 every other day until gone

Uecker, thanks for the detail list. I appears there have been no real changes to the med list we had compiled. Just a bit of a tweak to a date because of pred having first been given once a day with the night dose.
Mark your calendar for the next pred taper test on: Oct 7. To get the accurate and quick read, pain meds are normally full stopped on that date. On Oct 7 (with a total of 12 days on pred) our fingers will be crossed no pain to surface requiring another course of pred. Pred can usually do its job somewhere between 7 to 30 days (excluding any taper days).
What is the pain status now compared to 9/30 report of both pain and increased neuro diminshment wobbliness with the test taper of pred 5mgs 1x/day?
QUESTION Today, Oct 1, is there pain still? If yes, why has not methocarbamol been added and gabapentin moved up to 3x/day? These two pain meds are effective for 8 hrs, thus the need for 3x/day dosing.

Thank you Paula. He is doing well today. Back to acting like himself, and no pain shown. yay!! The wobbly is better too, standing up straight again.
He did have a couple pee accidents though, is this common w/ the prednisone? He hasn't had the accidents before. I am worried on that.
He did have 2 poops this morning, yay!! Still mid range majority of it , not hard like a rock, but not soft soft like playdoh either. the last one is normally softer then the rest.
Thank you so much for all your help!!

Uecker, very pleased to hear Goeff has full pain relief from Gabapentin 100mgs 2x/day while pred is at work to hopefully provide complete resolution of inflammation on the 10/7 taper.
Let us know if not only will pred be tapered 10/7 but also the pain-masking gabapentin 2x/day be stopped on same date.

We will see how today/tomorrow goes and update. Thank you so much for all your time and help it's greatly appreciated!!

When will pain masking gabapentin 2x/day be full stopped or back down in order you have a quick and accurate assessment about pain?
How's his poop?

10-10-23. Both vets said to start w/ the prednisone taper which we did, and to keep them updated on how he is doing. They may add in that methocarbamol if needed. He is doing better this time around, he is still able to walk like when he was on the 2x a day prednisone. No signs of the paw knuckling.

His poop is finally getting on the softer side, he went 3x today. We will see how tomorrow goes. Hopefully it will stay on the softer side and he wont get backed up again. The vets said once it gets to soft though, to discontinue the lactulose.
I checked around with vets in the area. We do not have any at home options for rehab/acupuncture etc. The rehab vet recommended to purchase a Assisi Loop to do at home?
Thank you so much for all your time and help, its greatly appreciated!!
