A few months back, Benji had an episode where in a matter of 24 hours, he went from a playful happy dog, to a dog who could not walk. He was rested (not strictly) given dex, methacarbonal and tramadol and seemed fully recovered in about a week. so when a similar thing happened the other week, I just started him back on the pain pills. He felt better quickly, so he went back to playing and jumping. Over the weekend, he was in a tremendous amount of pain, took him to the vet Monday afternoon where he was given a sustained release pain injection and a steroid injection, as well as a laser treatment. No improvement in 48 hours. Went back in today for another flasher and an X-ray. He is now knuckling and wobbling on the back end. Ordered to strict crate rest, MRI suggested. However, we would not do surgery regardless, so is an MRI even worthwhile? Benji is not eating hardly at all, so I think I will start him on Pepcid, which will give me yet something else to shove down his throat. We also made an appointment for acupuncture to tomorrow. I'm just so lost as to what to do for him, and it's really hard to see him so unhappy

Hi my name is Jen what's yours? I'm so sorry Benji is having a disk episode. Do you have Benji on 100% crate rest? Is he on any Meds? If so please let us know what they are, how often, and the dosage? The best thing right now is for Benji to be on 100% crate rest for 8 weeks in order for the disk to heal. It acts like a cast to restrain movement. My advice is to read as much as you can and do your reaserch. Healing prayers and thoughts headed your way!

Welcome to Dodgerslist Jen! Please let Benji know with your utmost confidence that things are going to be ok…because they will. With this disease self education is critical not just so you make sure the right things are being done for the best recovery but for your own emotions. The unknown is simply a scary place. Get ready to fight this disease now and in the future by knowing all things IVDD. There is no better place to start than on our main web page with "Overview: the essentials" and then read all you can as soon as possible. Here's the link www.dodgerslist.com/healingindex.htmThe best tool for healing short of when surgery is indicated is – Crate rest— 100% STRICT crate rest for 8 weeks. That means 24/7 in the crate except for potty breaks. It's lack of movement that heals..and with Benji in so much pain, he needs that spine protected from further injury.. No laps, no couch, no sleeping in bed with you, no meandering, scooting or dragging around during potty times. No baths, no Chiro, no VOM. The rest of the details of doing crate rest: www.dodgerslist.com/literature/cratesupplies.htmwww.dodgerslist.com/literature/CrateRRP.htmDo everything possible to preserve the spinal cord with good crate rest principals! The true success with IVDD is measured by the most important goal of returning your dog to a pain free, happy and full-of-love quality of life after crate rest. Should paralysis occur, please know that dogs do not view the lack of ability to walk as a minus as humans do. Dogs adjust to what is and then get on with the business of fully enjoying all that life has to offer until nerves repair! Tell your Vet that Benji is still in pain and see if some adjustments can be made to his pain medications.. Very important, as long as there is pain, healing can't begin.. Once he is pain free, you will start to see Benji's personality come back.. www.dodgerslist.com/literature/healingpain.htmPlease do ask your vet if there is any medical reason why Benji can't be on Pepcid AC. He is taking Dexamethasone which is the strongest of the steroids which can cause major GI problems, a real danger to his unprotected stomach. He may not be eating because stomach problems are developing.. Ask about Pepcid and also a second stomach protector Sucralfate that works in a different way to coat the irritated stomach lining.. Sucralfate is given on an empty stomach, an hour before eating. Pepcid 30 minutes before the Dex.. Take care and know we are here to help..

Just started Pepcid and doxy as well. Going to accupuncture today. He seems happy. I moved a crate into the living room and he ate a little tuna amd some peanut butter bread. Posture and walking is no better today, but he is still drinking and urinating on his own.

Can you specifically tell us if he pain is currently under control and has it been dose to dose of the pain meds (Tramadol and Methocarbamol)? Signs of pain are shivering, trembling, yelping when picked up or moved, reluctant/slow to move head or body, tight hard tummy. If he is still reluctant to eat, please also call in and ask for an RX for sucralfate in additiion to the Pepcid AC. Dex is well known in the medical community to cause severe GI track problems. The first red flag signs to take seriously are: not eating, vomit, loose stools. Read up on Sucralfate so you are are as educated about this med as you are each of his other meds: www.marvistavet.com/html/sucralfate.htmlWhat is "doxy"? Why was the anti-depressent Miratzipine prescribed? Can you confirm you ARE now doing 100% STRICT crate rest 24/7 only out to potty for a full 8 weeks? …. No laps, no couch, no sleeping in bed with you, no meandering around during potty times. No baths, no chiro (aka VOM). In other words do everything you can to limit the vertebrae in the back from moving and putting pressure on the bad disc

He is panting like crazy and whimpering...but I don't know if its pain, or stress from being crated. I am able to lift him and he doesn't yelp at all. He wants to move more, and tries to leap out of his crate when I open it.
I asked for Sucralfate and was denied, only was given Pepcid. I am going to a different vet this evening who is going to be doing acupuncture, so I'm going to ask her this evening. I had my horse on that a while back and it did wonders for him. He is not presently vomiting and his stools as of yesterday morning were normal (no stool since then).
Doxycycline is an antibiotic. Vet put him on that because IVDD is not confirmed, just suspected and she thought maybe he has some infection going on as well.
Miratzipine was prescribed as an appetite stimulant to try and get him to eat.
Yes, I stayed up very late reading last night. I did have him on my lap last night for a while, which was the only time he was calm and relaxed, but will not do that anymore so he can heal. No baths, no chiro. Just for going out to potty (on a leash, and carried out and back into the crate), and to the vet. Going to acupuncture tonight, and for another laser treatment tomorrow.
Please, if there is anything else, let me know as I will be going to another vet at 4.

I'm glad you are seeing another vet, a 2nd opinion… hopefully a vet that feels more comfortable in treating a disc episode. The hallmark component of conservative treatment is the 100% STRICT crate rest 24/7. Without crate rest, the disc does not heal, and a vicious cycle you have reported of pain, neuro diminishment, vet bills, more meds happens.
Panting can be due to Tramadol and due to steroids. Try placing a fan near his crate but not pointed at him to help gently circulate the air a bit.
Be aware you might be inadvertently training for unwanted behavior. To dogs rewards are: food, looking at them, talking to them, eye contact, approaching the crate, petting. So anytime you see unwanted behavior of whinning ignore it, turn your back, leave the room if you have to. Anytime your Benji is sitting or lying down quietly, give a reward. Soon he will learn which of the behaviors receive reward.
Dex is notorious for adverse side effects to the stomach. The answer is not stimulating the appetite, but to protect the stomach from damage. Miratzipine could have some pretty serious side effects. Do Google and read up on it.
What were the symptoms she observed that led her to believe an infection was going on leading to the prescription of Doxycycline?

Hi Jen,
My name is Yvonne and I am so sorry to hear about Benji. It is so heartbreaking to see your fur-kid in so much pain and unhappiness. My Cocker Billy just finished his 8 week crate rest and I am partial to Bichon and Bichon x's because I have a 13 year old Bichon as well that is my life! I get so unraveled when any of my fur-babies are sick so kudos to you for finding this forum.
Things will get better. You are a great mommy! Hugs to you both~~~~

She wanted the doxy since I was concerned he wasn't showing signs of improvement. The vet last night did not seem to think that was a good idea. So we opted not to give it [Doxycycline] to him.
The acupuncture seemed to help a little and we were also given some homeopathic remedies as well. He is eating little bits of people food - ground pork cooked in coconut oil as long as we hand feed it to him. Might tried scrambled egg later today. I feel better knowing he is at least getting something in his belly. I might skip the miratzipine as well today and see if he continues eating.
He is still panting a lot, and was a little tender this morning (which is probably because it was about 10 hours between tramadol doses. He is on the Pepcid now, just got his second dose. He held is urine overnight and had a long pee this morning. No stools yet. He is not really quick about righting his paw position was placed in a knuckling position, but 2 days ago, he didn't right the position at all. He is a little wobbly still on his back end, but it is not getting worse.
Will keep you all updated! I'm so glad I found you. I feel so relieved to know that there are so many of you out there to support!

Jen what are the exact names of the homeopathic remedies. All herbs, supplements are something to research and read about. Some are contraindicated with IVDD meds. I would refrain from adding oils or fats to his diet, while on Dex. If there is to be a stomach issue, diarrhea you do not want confusion of whether it is just due to diet change or whether it is a serious situation from Dex. A good option is low salt /no fat chicken or meat broth (boil up some hamburger or a chicken piece, cool and remove disc of fat) Add broth to his kibble. Microwaved and mashed sweet potatoe is easy to digest and nutritious. Best is if you can get him back on his kibble that has the full nutrients he needs for healing.
I would contact the vet from yesterday and strongly advocate for a 2nd stomach protector…sucralfate. The not eating is quite likely due to the DEX…. a big red flag. Pepcid helps to reduce stomach acids, but sucralfate works in a different way to bandaid the damaged mucous lining of the stomach and aid in repair. Both are often prescribed, my dog needed both.
Glad to hear you will stop the Miratzipine.
Was there are reason his Tramadol was extended to a 10 hour stretch rather than every 8 hours? Once pain arises it is harder to get back under control…so punctual doses are key to controlling pain. Tamadol can cause panting. Set up a fan near his recovery suite but not pointed at him just to gently circulate the air.
Benji is going in the right direction of nerve healing…with improvement of the knuckling. What is not yet right is his stomach protection (ask about a sucralfate Rx) and his pain control is not 100% dose to dose.
It will be much smoother sailing for you and Benji when his stomach is protected and full pain control achieved…. do let us know.

He won't go near kibble....and people food is only occasional as to whether he eats it or not. I will ask again for sucralfate. He goes in to vet #1 for laser again tonight...iThe tramadol being stretched was my fault. He was comfy overnight and I actually got a full night sleep for a change and slept past the 8 hours....
He is actually resting comfortably right now, no panting. We moved he crate into my daughters room overnight ( he usually sleeps in bed with her) and he was much more comfy it seemed.
I am concerned about what will happen when I go back to work in less than 2 weeks....he will be home alone for 11 hour stretches every day.

Can you get a friend or neighbour that you trust to help you out? Someone to give him his medication, and take him out for a pee while you are at work? People like that can be heaven sent while we are dealing with IVDD!

We stopped the mirazipine yesterday, and he is still eating. So that's good news. He isn't eating kibble, but did eat chicken for breakfast and pork for dinner. And he pooped tonight, which is great news.
He is still very wobbly on his back end, but doesn't fall over.
Hoping to see more progress over the next several days with more laser and acupuncture scheduled.

Were you able to advocate for a sucralfate Rx from vet #1 last Friday? The diet he is on is not balanced nor has the full nutrition he needs. It is ok for 1-2 days as a temporary thing. He needs to be transitioned gradually over several days to get back on kibble. The root of the problem of not wanting to eat needs to be addressed by protecting his stomach. Let us know how that goes.

No luck with the sucralfate from with either vet. Good new though, Benji ate his kibble this morning with a little chicken mixed in. His walking is a ton better and he only wobbled when he decided to lift his leg to pee. We moved his crate outside for a bit today while we were out and he seemed to really like that. He's resting comfortably, very little panting. Just some whimpering because he wants out of the crate. He is still on tramadol, every 8 hours on the dot, as well as methacarbamol. Pepcid 2x a day before his dexamethasone. Hoping he continues to eat his kibble, but today, for the first time since this episode started, I feel he will make. A full recovery. We are planning on rearranging some things to keep him safe in the future.

Benji is doing great! All normal eating and bathroom habits, and his personality has returned. He had laser yesterday and the vet was very pleased with his progress. We are starting to▼ wean off the dexamethasone, which he will take .25 mg 1x per day instead of 2x. All pain and muscle relaxers will stay the same. He immediately rights his position when his hind legs are placed in the knuckling position and is nolonger wobbling on the back end. Acupuncture appt tomorrow and laser on Thursday.
Should I be continuing the Pepcid 2x per day? Or just once before giving dexamethasone? When should pain meDs/ methacarbamol be cut back? Just wondering your thoughts so I am more informed when speaking to the vet.
Thanks again for all the support!

It sounds as though Benji really is doing very good indeed! Righting his knuckled paw is a very good sign. And no more wobbling - wonderful! You would only give the Pepcid 30 mins before the Dexamethasone, so if you're cutting the Dex back to once a day, then you would only give the Pepcid once a day. When you start weaning a dog off of an anti-inflammatory, you also should stop the pain medication. The only way you'll be able to tell if Benji still has pain when the anti-inflammatory is decreased is if his pain is not masked by a pain pill. This is what Dodgerslist says about weaning on their website: "The tapering off of a steroid is a health necessity that also allows a peek into the status of swelling. If pain returns, your vet needs to know and likely more time on the steroid would be prescribed. No signs of pain means the steroid has done the job." www.dodgerslist.com/literature/healingsweling.htmSo if pain medication is still being given, you would have no idea as to whether or not the pain has returned when the steroid is decreased. I think you should contact your vet with that question. Please let us know what he says.

As Marjorie says, that righting his paw and no more wobbling are very good neuro signs of nerve recovery! I agree also that to get a true picture of pain on the taper, please discuss tapering off or stopping the pain meds whose job is to mask pain (Tramadol and methocarbamol)
Literature does indicates there is a range of time in giving Pepcid AC… 1x a day or every 12 hours. Giving it every 12 hours means that it is available more consistently in the body at the stronger level. Also Dex is one of the strongest of the steroids meaning it stays in the body a longer time between doses. Since Pepcid AC use carries very little side effects, I would not hesitate to be over protective with continuing Pepcid AC 2x a day as it was prescribed until completely off of Dex.

I just wanted to update all of you who have been so helpful to me. Benji is doing very well. He is not confined overnight and when we are at work or away. But we do let him out supervised when we are home. He has been off all meds for a few weeks now. We have stopped laser and accupuncture, due to our work schedules. We started him on Vetri-Disc to help support his back at the recommendation of the acupuncture doc. I'm so grateful that you all had such great advice to help us along the way! He isn't exactly 100% yet, but he is well on his way!

Jen, the reason for 100% STRICT crate rest 24/7 only out to potty is for the task of giving the disc an opportunity to for good secure scare tissue. Oct 2 will be Benji's graduation day. Until then please consider giving him the very best opportunity to heal. Meds work to get pain and swelling down, there are no meds to let the disc heal. That happens only with time fo 8 weeks and limited movement. Reviewing this animation will help you see how important crate rest is. Right now Benji is like a chicken roasting in the oven. Take a chicken out too soon even when it looks brown on the outside and pink juices will run. Benji may look ok from the outside but his disc is not yet done…done healing.

Hi Jen, Delighted to hear that Benji is doing so well but i want to caution you about him being out of the crate for any length of time before the 8 weeks is up. His graduation date should be October 2nd.. "When the dog starts to feel better, he still needs to be confined to prevent further extrusion of the disk and sudden relapse with potential for profound worsening of neurologic status. " sturgisvet.com/smallanimalclinic/managingdiskdisease.html• "Strict confinement is crucial and is considered the cornerstone of conservative therapy for IVDD (the dog is kept in the cage at all times and only taken outside to relieve him/herself). In order to accomplish the goals of preventing further extrusion of the nucleus of the disk and to promote healing of the outer layers (the annulus) of the disk, strict confinement is necessary. Activity can easily push more of the nucleus into the spinal canal and dramatically worsen the neurological status. It is not uncommon to see dogs that have “gone down” after a short course of pain relievers given without insuring that the owner would enforce strict cage rest." Dr Larry Carpenter DVM MS ACVS sites.google.com/site/sturgisvet/small-animal-services/faq-s-about-disk-disease Wishing you and Benji the best..
