Josie had surgery 6/5 for a ruptured L1-2 disk that was 70% compressing her spinal cord. She was deep pain negative on the morning of 6/5. When we woke up she was dragging her legs and was in extreme pain. We rushed to our regular vet who determined she was deep pain negative and sent us to a neurologist. We did an MRI that showed she was a good candidate for surgery and the surgery was performed that afternoon.
Prior to this on Saturday night 6/1 Josie didn't come upstairs to go to bed and was restless - it looked like she had to poop but couldn't poop. We crated her all day Sunday thinking she had eaten something - a real possibility with a 5 year old dropping crumbs around the house. Monday 6/3 she still wasn't herself and we took her to our Vet. She diagnosed back pain and prescribed pain meds. She told us not to let Josie do stairs or play. She didn't tell us to do strict crate rest and she didn't tell us it was IVDD. I wasn't educated enough about back issues and I will forever wonder if we could have spared Josie the surgery if she had been on STRICT crate rest from Saturday.
Josie's surgery went well - they removed a lot of disc material and the next morning she could feel her toes again. Saturday 6/8 she came home. She has been comfortable and slowly regaining function every day. Sunday 6/9 she regained her ability to pee yay!!!! Then she started to be able to move her hips a little bit. I saw her lifting her leg at the hip to lick her foot. Now she can fully posture to pee. I don't think she can control her feet yet because she still knuckles all the time. She is so good about doing her passive range of motion exercises and is doing really good with her standing practice. I am hoping that after the staples come out we can start water therapy.
She came home on 6 meds... tramadol, gabapentin, rimadyl, pepcid, and 2 for peeing. We stopped the pee meds that first sunday since she could urinate on her own. We stopped the Rimadyl Wednesday 6/12 because she started throwing up. She is currently on 25mg Tramadol and 75mg Gabapentin every 8 hours. Her pain is very well controlled. I think we will try and stretch out the pain meds tomorrow and she if she is ready to go down to every 12 hours. I am thinking I may try and off set them so that they aren't wearing off at the same time. For instance give her tramadol at 10pm and 10am and the Gabapentin at 7am and 7 pm. Has anyone else tried that? Did it help with pain control?
Josie is being very good about crate rest. We got a new larger top open crate - her old one was too small to turn around easily in - and Josie LOVES it! I have gotten her some new toys to help keep her entertained and when she gets restless I do her therapy which tires her out.
While she is peeing on her own she is still wetting the crate every other day or so. I don't know if she just can't hold it overnight anymore or what. She is also getting a rash on her underside. It looks like diaper rash. I have been cleaning her off every time I change her bedding. I don't know what else to do to clear it up. The rash just started yesterday 6/15.
We are also in the process of making our house back friendly. We have a sunken family room so my dad is building ramps for Josie.
We go Friday to have her staples removed and a follow up evaluation at the Neurologist.

Welcome to Dodgerslist, Sarah. When dog begin to regain bladder control it may take a while to be fully able to void the bladder…so you might do a quick express check to make sure. Do you limit water at about 8pm and potty at 10ish…she should be able to have a dry night til about 6-7 am.
Talk with your vet about backing off pain meds if you believe all the swelling is now gone. Instead of every 8 hours try every 12 hours. IF you see any hint of discomfort not moving as well because it hurts, shivering/trembling… then you know she still needs her every 8 hours med dose.

Limiting the water - genius! Josie used to sleep in our room at night and wouldn't have access to water - that has got to be the difference. Now that she sleeps in her crate we have been leaving the water in there overnight - and I think she drinks more water than she used to because it is right there.
We did do express checks the first few days after she regained the ability to pee and her bladder was empty.
The vet instructions say at this point to give her the meds every 8-12 hours as needed for pain. In the last few days Josie has had a lot more energy and has been playing with her toys more - much more herself. Her incision is healed - the scabs are starting to come off which makes me think it may be time to do a test to see if she is ready to back down on the pain meds.

We tried going 12 hours between doses and Josie didn't seem to notice a difference. She was still comfortable and more energetic. We also took her water away before bed last night and we had a dry crate this morning. Hopefully that continues!

Glad to hear that Josie is responding well and her personality is coming back.. The dry crate in the morning was victory! Yeah!!

Josie started paddling/walking with her back legs while she is being sling supported! What a big milestone! She still can't support her body weight for more than a couple seconds but I am so happy to see slow progress How do I know when Josie is ready to taper off the meds? Right now she is on 75mg gabapentin and 25mg tramadol twice a day. I will run out of the tramadol on Sunday morning if I don't get more. She has been on every 12 hours since Sunday. I am trying to figure out if I need to refill her pain meds or if she may be ready to stop them. Do I stop one medication and just give her the other or do I stop them both at the same time? What have people had success with?

Hi Sarah, good question, and definitely one you want to discuss with your vet to be certain. With my dog, I tapered his meds around the same time post-op as Josie. In other words, instead of giving Tramadol twice a day, I gave it once a day for a couple of days, and then discontinued. I watched VERY carefully for signs of pain returning. Luckily, he had no pain.
During the taper of pain meds, signs of pain to watch for include shivering/trembling, reluctance to move, lack of appetite, panting, yelping, or a tight-feeling tummy (like a drum). If you see signs of pain, notify your vet and they likely will tell you to continue the pain meds for a longer course.

How is Josie getting along on the taper of her medications? If she shows signs of pain during the taper make sure the vet is aware so she can be restarted on her meds..
Hope she is doing well and she is adorable! LOVE her picture!

Josie has been weaned completely off her meds following the vets instructions. She isn't having any pain and is even rolling onto her back for belly rubs! She also took her first few wobbly steps today! I cried I was so happy for her. She is still tolerating her crate rest well even though she wants to be out and playing.

YOu have a great little girl to tolerate her recovery suite and how wonderful no meds and no pain. Wobbly soon will be steady… how is she doing today. What did the surgeon direct for PT… may she walk to and from the potty place for example?

Yes the vet said Josie can walk to and from her potty spot and walk around a bit as part of her PT. I have been doing her standing exercises and passive range of motion exercises with her too. She is starting to stand up when she eats her food. She still has a rash we are trying to get rid of. She is still on antibiotics but I have a feeling we will be going back to the vet to have it rechecked after we are done with the antibiotics. But overall Josie is doing great.

Josie graduates crate rest today! We are doing water therapy and her wheels from Eddie's Wheels will get here next week. Josie will be SO happy to go on walks again. She can hop and take a couple steps but most of the time still drags her legs but i know that will keep getting better with time. She is loving her drag bag for now and is sleeping in all her favorite spots!

Congratulations!!! With an Eddie's she can enjoy all the family activities again while nerves continue to heal. Keep us posted on how things are going and her progress.
