Hi - I noticed yesterday (1/1/20), Abby - approx 10 years old, 8 pounds - was yelping when we would barely touch her. Later in the day I noticed she was acting exactly like my other dachshund who had IVDD & a cervical disc rupture. Arched back, visible spine, resistant to or not able to shake head and ears, trembling. Not wobbly. We immediately took her to emergency vet (Blue Pearl) since it was 6pm. They did an exam - they didn't see any neurological issues, prescribed 4 weeks crate rest (I know it's 8 weeks & strict!) and they gave her: - prednisone 1 mg - 2 tablets by mouth every 12 hours for 4 days, 2 tablets every 24 hours for 4 days, then 2 tablets every other day until gone. - tramadol 50 mg tablet - give 1/4th tablet 2-3x per day as needed (we are doing 3x)

I talked to my vet today and told her I was worried about her in her crate - as she has separation anxiety. In the last 4 months she's had TWO dentals to remove several broken teeth. They were going to add in gabapentin (do not know dosage yet - picking up tomorrow)
As I've gone down this road before several years ago, I asked the emergency vet about pepcid, and she said we didn't need it. (?? is this right? ask my own vet??)
She seemed worse (pain) when I got home from work today than she was last night or this morning. My mother in law is going to come over to give her pain meds right at 8 hours tomorrow, since it was about 10 hours today.
Is she on enough pain medicine? OK to add in the gabapentin?

Christine, welcome to the Forum. Glad to hear you understand the crate rest is to get the disc to heal and that takes 8 weeks! Wonderful to have the assist of your MIL in giving the meds on an every 8 hr schedule. It is clear that Abby is way under medicated for pain! When the pain meds are correct pain is in control in 1 hr or less and it stays in control dose right up to next dose. Blue Pearl should have someone there tonight to read her file and get those meds right. --- neck discs just ARE more painful and an aggressive pain med approach should be impletented. --- Gabapentin for nerve pain every 8 hrs. --- methocarbamol for muscle contraction pain every 8 hrs --- tramadol as the general analgesic also every 8 hrs. The usual in controlling pain is to Rx at the higher end of the mg range AND promptly every 8 hours. So for Abby that may mean close to a full Tramadol tablet, not the minimal 12.5 mgs of tramadol Rx'd. See Plumb's reference below so you can advocate for adjustment. Plumb's is considered the "drug bible" of the veterinary world. -- Extra things you can do at home to help with a neck disc. BTW, did the vet confirm it was a neck disc? These tips won't hurt even if it is a back disc: www.dodgerslist.com/literature/cervical.htmACID SUPPRESSOR a must with Prednisone! Proactive vets don’t wait til there is lip licking of nausea, not eating, vomit, diarrhea leading to serious bleeding ulcers, red or black blood in stool due to the extra stomach acids anti-inflammatory drugs cause. Pepcid AC (famotidine) blocks the production of acid. The usual dose of Pepcid AC (famotidine) with a disc episode is 0.44mg mg per pound every 12 hours. Pepcid AC has a very limited potential for side effects. Ask if your dog has any health issues to prevent use of Pepcid AC (famotidine)? (doesn’t need it, we wait til there is problem…are NOT answers to your question!) If you get a “no health” issues answer, then go to the grocery store to purchase over the counter Pepcid AC containing one single active ingredient (famotidine).
CALMERs YOu may need to help Abby to be able to relax in her recovery suite. Using any oral calmer in combination with a Pheromone diffuser seems to work best. It takes several days for these to start working - it isn't immediate but they are a much better option if you can avoid heavy duty prescription sedatives (ACE, alprazolam or trazodone). Of course always keep your vet in the loop on all things you give your dog. Place a DAP pheromone diffusor at floor level where the recovery suite is: --DOG Adaptil (DAP) wall plug in diffuser 48ml www.adaptil.com/Use diffusor with one oral calmer from below: Oral calmers: 1) ANXITANE® S chewable tabs contain 50 mg L-Theanine, an amino acid that acts neurologically to help keep dogs calm, relaxed www.virbacvet.com/products/detail/anxitane-l-theanine-chewable-tablets/behavioral-health 2) Composure Soft Chews are colostrum based like calming mother's milk and contain 21 mg of L-Theanine. www.vetriscience.com/composure-soft-dogs-MD-LD.php 3) Rescue Remedy is a liquid herb combo to help with relaxation www.bachrescueremedypet.com RECOVERY SUITE A wire crate is often preferred over the plastic airline type of crate because wire gives better visitity and feels less isolating. An expen is also anothe type of suite. Some dogs do well in a pack N Play, too.

Hi - thank you for the quick reply! I got the meds straightened out with my vet, ordered the calming pills and diffuser - will get set up today. She increase in medicine [?] was just what she needed.
I kept telling the emergency vet she was showing the exact signs as my other dachshund with a neck disc rupture but she kept saying “back” to me. Outside of the neurologist, I’m not confident in any of them anymore. They gave me the wrong number of prednisone pills - gave me 20 instead of 28. When I got home I was scheduling out the medicine & days on a calendar and the written RX and # of pills didn’t add up. 🙄
I’m very worried that only 2/3rds of the neck injuries heal with conservative treatment. My other dog had surger at age 5 & then died the day after we took her to Blue Pearl for another rupture at age 11.. we assume a spinal cord injury. Abby is at least 10 - she was a highway rescue doggie in 2011. Surgery is just not something we can handle, especially at her age - and after the $2500 in dental care she has had this year. ☹️☹️☹️
I’ll keep you posted on her recovery.

Christine, if the pain is indeed fully under control round the clock, right up to the next dose of pain meds (gabapentin and tramadol) then Abby has a very good chance to heal her disc in comfort while the prednisone works on getting all the painful inflammation resolved. To prevent delay in advising, so we are prepared and at the ready to provide good comments when needed, 1. Would you bring us up to speed on what is her actual med list is now. The concern is that since there is a taper to begin tomorrow (SUN 1/5) but pain has only just now been brought under control. Likely all swelling would not have been resolved by Sun (with that rather short 4-day pred course) Recall that it often takes in the range of 7 to 30 days (excluding any taper days) to resolve all the inflamed tissue). 2. What is the Plan B with your vet if pain were to surface on Sunday's start of the taper when your vet is closed? Do you have extra pills to back up to the orig anti-inflammatory pred dose OR do you have a script you can fill at a local 24hr pharmacy? Hi - Today was the start of the taper, but I have plenty of prednisone to last until I talk with the vet Monday morning (1/6)
8 lbs /3.63 kgs Prednisone started 1/1- as of 1/5: will keep at 1mg - 2 tablets by mouth every 12 hours for additional days since pain is just now under control - then taper in this method: 2 tablets every 24 hours for 4 days, then 2 tablets every other day until gone.
tramadol 25 mgs 3x/day (started 1/1 at 12.5, increased on 1/3 to 25) gabapentin started 1/3: 25 mgs 3/day Pepcid AC started 1/3: 5mgs twice a day with Prednisone
started Anxitane 1/2 tab twice day 1/4 - I noticed ZERO difference yesterday or today. Plugged in diffuser, noticed no difference there either. She's still whining and upset she's in her crate when awake.
Also left this off the list - she's been on prozac 5mg per day for months.

I have enough gabapentin to last about 17 more days at current dosing. I have enough tramadil to last 22 more days at current dosing. I have enough prednisone to last 3.5 more days at current dosing - will have vet refill Monday morning as it often takes in the range of 7 to 30 days (excluding any taper days) to resolve all the inflamed tissue).

Christine, the calmer can take a couple of days to reach peak. Still there are some dogs that just do need an Rx item to help them be able to relax. Still with Abby being on two drugs (prozac and tramadol) already that allows more serotonin to accumulate, the vet will be quite aware to take that into consideration before adding another drug that could cause Seratonin Syndrome. It is always a good idea to read up on each med your dog takes to be in the know and best able to ask your vet pertinent questions. Consider some of these other calming ideas: -- Many members have found a pet stroller to solve the whining problem because the stroller can be wheeled from room to room as you go about your activities. Pet strollers, however, should only be used when you are directly supervising. More details on strollers: www.dodgerslist.com/literature/strollers.htm-- Put a garment you have been wearing and have not washed in the crate. -- Nan Arthur, CDBC, CPDT, KPACTP: According to the book "Stress in Dogs" by Martina Scholz & Clarissa von Reinhardt, the most well-behaved dogs get 17 or more hours of rest and sleep per day. Teaching self-calming exercises can also help your dog to relax more. You can make something as simple as eye contact a very rewarding behavior that also acts as a way for your dog to “ask permission” when he wants something. When dogs have a focus and an understanding about how to behave to get what they want, they are much calmer overall. To do this, each time your dog looks at you, say, something like, “Yes!” or use a clicker to mark the second he looks at you, and then give your dog a high-value food reward. [NOTE: for crate resting dogs, shift your body a bit] .... wait for your dog to look up at you again, say, “Yes,” and reward again. Do this exercise 10 or so times and then say, “All done,” and put the treats away. Come back later and do it again until you can see that your dog is really starting to make automatic eye contact in hopes you will say, “Yes,” again and give him his reward. [NOTE: treats should be subtracted from the normal daily kibble ration so as not to gain weight during crate rest.] -- At night, try placing the crate on a sturdy bedside table to sleep next to you. -- During the day try the coffee table or the dinning room table so there will be a view out a window and a better perspective on what is going on in the house from a high. -- Make the crate or ex-pen more cozy by draping a blanket over part of the top. -- Relaxation Music for Dogs! Thank you for sending all of those tips and ideas.
I can not place a blanket over the crate- she will chew on it and pull it into her crate. I've never been able to drape anything over her crate, or place it near any hanging fabrics of any kind. I can try wearing and leaving her an old t-shirt for comfort.
I called my vet today to request a longer period on prednisone (I gave all the reasoning you listed), but the nurse told me the vet wanted me to continue with the taper as planned, but to let them know if I noticed any regression at all. So today will be the first day to go down to two-1mg every 24 hr for 4 days.
Last night she did a lot less whining... I hope it's progress!

How is Abby doing on the taper today, Christine? Any sign of pain returning or any worsening of neuro function?
With pain meds on board, it will be difficult for you to quickly determine pain/swelling and the need for the return to the anti-inflammatory dosage of Prednisone. Usually a vet will also taper or stop the pain meds when the taper of Prednisone starts. It's important to know whether there is still swelling pressing on the nerves of the spine as it's that pressure that can cause nerve damage.
Prayers that Abby does well on the taper of the Prednisone.

Hi - we will be tapering the pain medicine tomorrow. My vet did not give instructions on the taper except for the pred. What should I expect to hear on the taper for the gabapentin & tramadol? tramadol ▲25 mgs 3x/day

Still on full pain meds as in previous descriptions: Abby seems in a much better mood. Lots of tail wagging for me, my husband, and my mother in law. She's shaking her head/ears like normal. However, a couple times (not every time she is out to potty) I have noticed that she seems a little drunk. Like she will side step or just losing her balance. I assume by tapering the tramadol and gabapentin I'll be able to see if it's a medicine side effect vs. nerve damage?
editing to add: my husband and I just let her out to potty. She didn’t seem drunk at all. Looking back, we think she was just excited to be out and with us.

For example with tramadol your vet Rx'd it at either 2x or 3x/day. So you could go down to tramadol 25mgs 2x/day as a backing off of it. Same idea with gabapentin. Let us know what you do re-backing off of pain meds so we have a record. Would not hurt to call and specifically ask your vet. A vet will tell you their preference: 1) either full stop of all pain meds OR 2) backing off and what that means in terms of down to dose in mgs and/or how often to give.
When backing off of pain meds you would be monitoring for re-surfacing pain. When taper pred you would be monitoring for any worsening neuro function from the point she is at the start of the pred taper.
As damage to the spinal cord increases, there is a predictable stepwise deterioration of functions. When nerve healing begins, often it follows the reverse order. 1. Pain caused by the tearing disc & inflammation in the spinal cord 2. Wobbly walking, legs cross 3. Nails/toes scuffing floor 4. Paws knuckle under 5. Weak/little leg movement, can't move up into a stand 6. Legs do not work at all (paralysis, dog is down) 7. Bladder control is lost. Leaks on you when lifted. Can no longer sniff and then pee on that old urine spot outdoors. 8. Tail wagging with joy is lost 9. Deep pain sensation, the last neuro function, a critical indicator for nerves to be able to self heal after surgery or with conservative treatment. If surgery is not an option (for whatever reason) then the best option is conservative therapy.

Hi I noticed yesterday Abby was panting a bit in her crate. She was panting again this morning, as well. She was trembling, but she's been a trembling dog her entire life.
We ended the prednisone taper yesterday. She is showing no other signs of pain/neuro damage. She's pottying well, shaking her ears, wagging her tail. No crying/yelping, no arched back.
Any advice? tramadol 25 mgs 2x/day gabapentin: 25mgs 2x/day Prozac 5mgs 1x/day

As previously mentioned, having pain meds still on board makes it difficult for you to determine whether there is still pain/swelling or not. If there is still swelling, then there is still a need for the original dosage of Prednisone for a bit longer. It can take 7-30 days for the swelling to resolve and until then, then anti-inflammatory dosage of the Prednisone is needed. It sometimes take longer for the swelling from a neck injury to resolve. Once the swelling is gone, the pain will be gone, too, so it's difficult to understand why your vet still has Abby on pain meds which may no longer be needed. Please speak to the vet regarding the stopping of the pain meds so a true test for pain can be made. Panting can be a side effect of the meds. A fan near but not pointed at the crate will help. Also a rice sock from the refrigerator can help them cool by laying their tummy along side of it. Fill a sock with 1-2 cups of rice and tie the end of the sock closed. Try a frozen broth ice cube to lick on. Or it can be from anxiety. Or it can be a sign of pain. If a fan helps, then it is most likely a side effect of the meds. If you see other signs of pain with the panting such as shivering, trembling, yelping, tensed up tummy, then the panting could be a sign of pain.
