[Original subject line: Cervical episode] Milo, 12 yo doxi, 11.5 lbs had an episode in cervical spine[8/20] early morning, Whelping and painful. We went to ER. Was given hydromprphone and cerenia while at the clinic. D/c home with caprophen 12.5mg q12h and gabapentin 50mg q8-12h. We already got famotidine. He was given hydromorphone about 7 hrs ago and gabapentin and caprophen hour ago. He's pretty sedated and out of it. Is it normal? I'm so worried about my lil guy. Please help me get him better. I'm sorry to hear that Milo is having another disc episode, Kate. Cervical episodes can be more painful. You're one step ahead in that you already know how to do correct strict crate rest for a full 8 weeks. Take it one day at a time. Milo is in good hands! As for Milo being "pretty sedated and out of it", he could have been given a very high dosage of hydromorphine and cerenia at the ER and that may not have been out of his system when the at-home meds were started. If you have concerns, you should reach out to the ER and let them know. With a cervical issue, there are a few other things that should be done, such as raising the food and water dishes so your dog doesn't have to bend neck, moistening hard kibble to soften, etc. dodgerslist.com/2020/05/05/cervical-care-tips/Is there still currently pain? With the injections at the ER this morning, Milo may not be showing signs of pain. However, do keep this question in mind and let us know if the at-home meds are keeping pain completely under control, with no sign of pain from one dose of meds to the next. Cervical episodes can be more painful than back episodes and Milo is only on one pain med - Gabapentin. Should pain arise, contact the ER and advise so pain meds can be adjusted. Most vets will prescribe three pain meds to get the severe pain of cervical discs completely under control: Tramadol as a general pain med, Gabapentin for nerve pain and Methocarbamol for the pain of muscle spasms which often accompany cervical episodes. Signs of pain: ☐shivering, trembling ☐yelping when picked up or moved ☐reluctant to move much in crate such as shift positions or slow to move ☐tight tense tummy ☐can’t find a comfortable position ☐Arched back ☐ Holding front or back leg flamingo style not wanting to bear weight ☐head held high or nose to the ground ☐Not their normal perky selves? Milo is to take Gabapentin 50mg every 8-12 [which?] hours, #pills 30 Caprophen 25mg give 1/2 pill q12h, #pills 7 [total 7- 12.5mgs 2x/day doses=7 day course] Famotidine 10mg, 30 min before each Caprophen??
I called er because I was very concerned with Milo being sedated and was told it was to be expected. It's wearing off now. He's not yelling in pain but has gotten his 2 doses of gabapetin when beck at home.
Why does he twitches? He has these involuntarily jerks of front and hind legs a few times a minute. They are very subtle. Is is something concerning? 😞😒
What to you think about CBD for dogs in IVDD flare up? I don't know what else I can do? I'm so worried 😭 I work from home and Milo has my attention at all times. It's only two of us. Please help me help my puppy 💔

Kate, very good to hear gabapentin alone currently is blocking pain and providing full relief from pain round the clock, night and day. FYI, often neck discs require each of the 3 sources of pain to be covered by three different meds: √ Gabapentin f or nerve pain 3x/day __methocarbamol for muscle contraction pain 3x/day __traMADol as the general analgesic 3x/day While you got an Rx for Gabapentin 50mg either 2x(every 12 hrs) or 3x (every 8 hrs) a day, we need to know which have you been actually giving? Are you keeping a med chart to note patterns with doses, etc.?

Thank you so much Paula. I'm very stressed out and keep crying and I think it distressing Milo too. I've been watching him like a hawk. I don't know what else to watch as far as pain is concerned, comparing to yesterday he's definitely better but again, he takes meds. He rolls on his back and wants me to rub it. I don't encourage it. He's does it because this is what we do. We doesn't pant or shiver. He wags his tail when I speak to him. He got excited and moved too vigorously and yelped one time earlier today. I got really worried. Is it normal he still has healing? I have an alarm on my phone and hive him his meds as prescribed. I'm very frustrated because thr vet hasn't called me back yet. I called the first thing this morning. I want to ask for the other medications.
I'm going to call ER again but I don't know if they will prescrive them to me. Can I obtain a prescription on line somewhere and have ot send to my pharmacy even if I have to pay for the virtual visit? Do you have any recommendations?
I just hate my baby suffering. I breaks my heart into pieces. Why haven't they prescribed those other meds yesterday at er?
Please continue educate me and provide recommendations. What should I keep on eye for? I feel so helpless right now. Than you

Your vet would take into consideration weight, any health issues and prescribe the correct dose. Look for the Rx to include 3x/day dosing for pain medications. Pain meds last for 8 hrs. I don't think people's doctor is qualified nor licensed to prescribe meds for an animal. I know when worried about our pets, it is hard to not be distressed. If you find you must cry do it outside where Milo doesn't see or hear. He will think something is not right and might even think he's the cause! You report GOOD news...Milo is definately better than yesterday. If he can roll on his back that definately means the pain meds are controlling pain. It is only the carprofen test stop that reveals if all painful swelling is really gone. Heck it might be gone today for all anyone knows. You WILL have an answer starting Sun 8/27. And if another carprofen course is needed, that would not be unusual. Observing two or more signs of continued pain, would help you to confirm that pain is not being controlled by the current single pain med, gabapentin. If you heard him yelp, due to too much movement, then you will want to PHONE the ER vet and see about methocarbamol. IF it was only one time yelp and he is calm and relaxed now, WHEN you can, discuss methocarbamol for being proactive in addressing any potential pain. Transports can be too much of a risk for early healing disc. Vets who know IVDD understand, and typcially adjust meds over the phone. If your vet has not yet seen Milo for this disc episode, then I believe he would need to examine him before presribing any meds or adjusting them. Your best bet is to work with ER to get the vet on duty to look at Milo's file and be able to adjust meds for full control of pain and potential pain. Do know Milo's body IS INDEED healing the disc. It's gonna take 8 weeks of limited movement to get the job done. FAMOTIDINE for a 12lbs doxie should be: 5mgs 2x/day. Famotidine is an antihistamine (contains no mineral salts of aluminum, calcium or magnesium). Famotidine suppresses production of stomach acids. Once started, keep with the every 12 hour dosing. Famotidine lasts for 12 hours. We consulted with the Neuro surgeon who supports Dodgerslist. This is what he told us:

Milo received Hydromorphone and cerenia while ER on 8/20. He was started on Caprophen 25 mg 1/2 pill q12h on 8/20 and has had 3 doses so far given religiously q12h. He started Gabapentin 5mg q8-10h on 8/20 and given religiosity q8h. Famotidine 10 mg 30 min prior to Caprophen started on 8/20. I was instructed ▼ famotidine should be administered as 5mg bid. I am going to request that vet prescribes methocarbamol and tramadol as recommended but it's still pending due to three vet hasn't called me back yet. Hopefully i can start Milo ok these tomorrow. When I spoke with the assistant, she said it should be no problem since he was taking these meds when he had his last episode. He's not presenting any new pain manifestations. He's looks relaxed and taking a nap after his last dose of gabapentin and caprophen. He still twitches [ask the cause]. He doesn't look like he's in pain when he does but I can't know for sure. I will definitely address it with the vet. Is it OK I'm hoping he may actually get better with conservative Tx? I just want my baby better. I wish I could bare his pain for him He drinks water ans eats. This is good, right? Yesterday 8/20 he didn't want to eat od drink for a whole day I was extremely worried. Kate, there was no recommendation to add pain meds UNLESS you feel Milo is suffering with pain. The FYI mention of the 3 usual pain meds, was in light that it seemed you were reporting Milo was NOT IN ANY pain with gabapentin alone. The FYI was to show you how pain free Milo was on just one pain med, when most dogs need three pain meds! I should have written that in a more clear way. Of course discuss all your observations with the vet and you both come to a conclusion what meds should be adjusted, if any. There was thought, with the twitching to discuss if muscle relaxer (methocarbamol) might be of use. And of course to get the vet's take on what might be the cause? DUPLICATION of med useWe keep an updated med list right in your post each time you mention a change to a med. You can help us out by not repeating what we already know. It takes much time to cross check whether there has been a change. ONLY report when a med has been changed (stopped or added) with date, mgs and times/day. Thanks, that will really help us out. FYI, these two meds are no longer on Milo's meds list because: --- Hydromorphone given on 8/20 lasts for less than 8 hrs. It is stronger than morphine. That could be the reason for the sedation you reported on 8/20. --- Cerenia given on 8/20 to prevent nausea/vomiting lasts for 24 hrs. MONITORING NEURO FUNCTIONS during a disc episodeIf you were to have an understanding of what a decline looks like AND to see where Milo is, I think you will have a calmer brain and less anxiety. Milo is a VERY good candidate to heal with conservative treatment, if you continue to be conscientious re: limited movement of his neck and back. If there would be increased damage to the spinal cord, it happens in a VERY predictable stepwise deterioration of functions. Excessive movement of the back/neck is the usual cause for a decline. 1. √8/20 Pain with initial tear of disc and ensuing swelling around the spinal cord 2. ___ Wobbly walking (front or rear limbs) ___ legs cross (front or rear limbs)3. ___ Nails/toes scuffing floor (front or rear limbs)4. ___ Paws knuckle under. (front or rear limbs) ___ Dog is slow to correct or can't right the paw(s) at all5. ___ Weak/little leg movement, unable to reposition in recovery suite ___ can't move up into a stand (front or rear limbs)6. ___ Legs do not work at all (paralysis, dog is down)7. ___ Bladder control is lost. Leaks on you when lifted. Can no longer sniff and then pee on that old urine spot outdoors.8. ___ Tail wagging with joy is lost9. ___ Deep pain sensation, the last neuro function, a critical indicator for nerves to be able to self heal with conservative treatment or after surgery. Thank you Paula. I'm very sorry for sending a duplicate medical message. I have this pop window asking for medications and I thought I had to reply to it. My apologies for taking away your time. I spoke with the vet and he said Milo definitely needs more caprophen tx and will get more medications and then we'll re evaluate. I addressed the twitching and the dr said methocarbamol will help with that. He will also prescrib tramadol in case Milo needs it. I have been monitoring Milo for pain and I certainly don't want to over medicate him I'd his pain is controlled with gabapentin. I will send a medication update as soon I get medication and instructions. Milo is stable and resting. Thank you for sending ivdd materials. I have a question, Milo wants to give me kisses and lick my face. Can I let him or it'd hurt him. Thank you

Kate, allowing Milo to give you some kisses would be an excellent spirit lifter for him and you! As long as he is not rambunctious in his kissing, it is ok.

Good morning, Milo has been prescribed additional medications : methocarbamol 500mg,, quantity=15, give 1/4 pill q8h will start today on 8/22 Tramadol 50mg, quantity=21, give 1/4 pill q8h, prescribed on 8/20 and not stared as of yet due to Milo's pain seems to be controlled with gabapentin. I got more caprophen, additional 30 pills and gabapentin another 30 pills. The vet said Milo has to finish caprophen and will be reevaluated. Milo is on 4 medications: gabapentin, caprophen, methocarbamol and famotidine with no changes to original doses or frequencies.

Good news that Milo is stable and happy !
No one wants a dog to be on more medication than necessary. If gabapentin alone is keeping Milo's pain under control, no other pain meds are needed. If you start to see any signs of pain, do not hesitate to give him the other meds as prescribed.
It is okay to give the meds at the same time unless the vet has said otherwise.
Cold laser can help start nerve healing during an IVDD episode. Just be careful not to put too much pressure on the spine when using it.

Thank you so very much. I started to relax a bit. It's so healing to see my puppy (he's an old puppy) smiling and looking more like his normal self. I've been extremely careful with him. I have a question about feeding Milo. We've been pretty active. We used to brisk walk for and hour and a half daily. I don't want him to gain wait. Should I give him smaller portions since we have zero activity now? Also his poop is a little loose. Is it a side effect of medications? I don't think he has a tummy ache. He's doesn't throw up. Thank you!

Kate, for most dogs, Famotidine alone will do a good job of protecting the GI tract lining from the damage carprofen can do of extra acids. When that is NOT the case, then call and ask for an Rx to Sucralfate to be added to the famotidine. It works in a different way than famotidine. Know all about sucralfate, its timing with food and with famotidine: marvistavet.com/sucralfate.pmlDogs do not speak up at the first signs of nausea like a person would. This is the usual progression of stomach damage signs from extra acids caused by carprofen & change in routine: lip licking, nausea, loose stool, vomit, bleeding ulcer, red or black blood in diarrhea, perforated stomach lining. Right now Milo's body is busy with repair jobs that need fueling by good nutrition. Now is not the time to go on a diet. Make sure you are not offering any high calor snacks. Instead give lo-cal treats such as small piece of carrot, apple.

Thank you very much Paula. I will call the vet [8/24] tomorrow and see if he can prescribe it. My Poor little guy, I hate having him go through all this 😭 I've been concerned with Milo scratching and digging in his bedding/blankets. I try to district him when he does it because it looks like it's something that can definitely hurt his neck. He also rolls on his back and wiggles around. He normally does these things and these aren't new behaviors. Should I discourage it while he'shealing? I'm also very curious of what the repairing provecess looks like in his spine, what is happening now? Do you have any information for someone with no MD or DVM training? Thank you

Kate, --the spine is made up of small bones (called vertebrae) which enclose the spinal cord. -- Discs are located in between the vertebrae. When an animal walks, moves the vertebrae move. Healthy discs provide a cushion between the bones. A bad disc can no longer cushion. Instead when spine moves, it pushes on the bad disc. Causes a painful bulge (or worse) into the spinal cord. -- It is said 1 in 5 Dachshunds can be born with IVDD: inter (between) vertebral (vertebrae) disc disease. This is a disease of prematurely aging discs. If a dog is born with this diease the discs are already prematurely aging at AGE one YEAR OLD!. However it is not typically til they are 4-7 years old the owner and the vet first learn the dog has that disease with the first sign of pain, possible leg weakness, etc. WHAT IS HEALING?During conservative treatment, --- The strict rest is what allows the damaged disc to self heal by forming secure scar tissue. No meds heal a disc. --- The anti-inflammatory, carprofen, is used to reduce and finally resolve all painfully inflamed tissue surrounding the spinal cord. Usually that process takes 7-30 days. --- Pain meds are used until it can be prooved by the "test stop" of carprofen that all tissue is no longer painful swollen. Thank you so much Paula, this is extremely helpful. I will definitely print the Roadmap! It's so sad so many doxies have this disease 😟 It's so unfair. Is there any research in place? Is anyone working on finding a cure or reliable prevention? My doxi has never walked up or down the stairs, I have ramps and rugs on floors. Never played tug of war, never has been picked up wrong. He's been on glucosamine his entire life. I just don't understand how he's gotten his 3 episode already. And i understand he may get more and not make it 😭 It very heartbreaking 😣
Would you please address his gigging in his blankets and wiggling around? Should I be stopping it? I have been but it doesn't take much for another injury. Thank you

It is definitely sad that so many dachshunds, and other dogs, have IVDD. There is research being done. I know the University of California Davis is researching genetic factors of the disease. Looking at Google, there is other research being done also. A breakthrough can't come soon enough.
IVDD is a disease our dogs are born with and despite our best efforts there may be a disc episode.
If you have concerns about Milo wiggling around on his back, you could try to redirect him with a toy or game. Wiggling like this does not cause him pain, or he would not be doing it. I don't think it is a cause for concern.

Thank you so much Romy. I've been undereducated and I appreciated this postal and all the materials so much. I was under the impression it's only dachshunds and beagles, and these other longer pups with huge wobbly ears. This is so sad. I feel like I live on the edge since his episode when he was 5. I'm sure I'm not the only dog's mom on here.
I can't thank you enough for your support and empathy.
I was unable to get the additional medication for the stomach that Paula suggested because my vet is closed on Wed but I'm happy to say Milo's poop was so much better [8/23]this morning!! I just want him to be comfortable 😞
When is a good time to consider spreading gabapentin more? Milo is still on 50mg g8h. I'm not trying to rush things, I'm just wondering what the recommendations are.
I was laying next to him last night and he still twitches but I want to say every 10 -15 minutes now while before he did every few seconds which was heart breaking. Is is a good sign?

Some breeds, like Dachshunds, are more prone to IVDD, but here at Dodgerslist we have seen many different breeds. My own dog, Frankie, was a pit bull. When Frankie went down, I had never even heard of IVDD. I learned everything I could, most of it from Dodgerslist. It definitely helped me feel more in control of the situation.
I am glad that Milo's poop was better this morning. It is still important to speak to the vet about a stomach protector to avoid stomach problems caused by excess acid. This is a side effect of all anti-inflammatories.
It would be best to speak to your vet about cutting back on the gabapentin.
The twitches coming less frequently is good news. If they seem painful for him, let the vet know. Milo should not be in pain.

Hello! Milo is doing really well ❤️ It seems like his appetite is back to normal. He no loner twitches.He's still on the same medications. I was wondering when it would be a good time to start weaning him him off of Gabapentin and Methocarbamol. He gets both every 8 hours. Should I try spread them out more? Every 10 or 12 hours? Please provide guidance. I don't want ot over/umdermedicaid him. He still takes caprophen and famotidine as prescribed. Thank you very much.

Great news that Milo is doing well, Kate! That's wonderful.
It can take 7-30 days for the swelling that's pressing on the nerves of the spine to resolve. Carprofen is scheduled to be test stopped on 9/11. You should speak to your vet prior to that date to determine what the plan is when Carprofen is stopped. Does he wants you to gradually wean Milo off of the pain meds prior to 9/11 or should all pain meds be stopped when Carprofen is stopped? The stopping of the Carprofen is a test to see if all swelling/pain has resolved. If so, then all meds can be stopped. If not, then a bit more time on all meds is needed. Having pain meds on board when Carprofen is stopped would make it difficult for you to determine whether swelling/pain has resolved as the pain meds would mask any pain that would alert you to continued swelling. At this point, continue with all meds as prescribed until your vet tells you otherwise.

Kate, a stroller is often used during the 8 weeks of STRICT rest as a recovery suite which to keep a dog by your side as you move about your home. Used on the home's smooth flooring/carpet, an even if you sit on your deck to take Milo with you. dodgerslist.com/2020/05/17/pet-stroller-conservatve-treatment/Right now the focus is on getting the disc to heal. Taking Milo where he might meet other dogs, would likely cause him to be too rambunctious or barking with gusto. Best is for Milo to stay indoors in your hospital (your home). It looks like on Tues 9/11, you will the first opportunity to begin assess if any painful inflammation still exists with the stop of Carprofen. The stopping of pain meds is the ususual. However for certain patients, a vet may prefer to back down the pain meds. Remind us which does your vet want on 9/11 so as not to be blindfolded about pain : full stop of all 3 pain meds OR to back them down?After crate rest, if the dog has weak legs or they don't work at all, a pet stroller can be safely used on the more uneven terrain of outdoors.

Thank you very much Paula. I understand the reasoning behind the stroller.
The [9/1] vet said I can start gabapentin and Methocarbamol every 12 hours. Caprophen and famotidine still the same, every 12 hours and total of 3-4 weeks.
I will check with thr vet prior to 9/11 if I should stop all meds all together or continue with Caprophen + famotidine for another week from 9/11. I'll start 9/4 gabapentin and methol q12 hours starting tomorrow. Thank you. I'm very nervous. I hope he's healing and that he's pain won't start manifesting with lower frequencies 😑 Thank you

What date did the vet tell you to lower gabapentin and methocarbamol, was it today, 9/3 or ....?
Carprofen leads the way in determining when pain meds reduce.
Typical with a disc epsidoe is to NOT lower pain meds during the use of the anti-inflammatory carprofen. When carprofen stop, then pain meds stop/reduce. Carprofen's job is to get rid of all painful swelling in a range of 7-30 days (1 week - 3 weeks). Pain meds enable Milo to heal in comfort from pain. When Carprofen stops, THEN it is time to also stop/reduce pain meds so that YOU at home can observe if any pain still exists.
Help us make sense of what you have conveyed. Why jump the gun to 9/4 in reducing pain meds a week before carprofen stops? If the vet is now guessing carprofen maybe has done its job, then why wouldn't the carprofen stop date also changed to Monday 9/4 along with reduction in pain meds?

Kate, you know how meds are typically used during a disc episode. If this vet requires an office visit Mon 9/11, then a visit should be at new-IVDD-comfortable vet that you hire and risk a trip in for.
If you are staying with this vet, then you will go by their directions.
See what happens regarding Milo's pain if you go to vet-approved gaba and metho every 12 hrs dosing. At some point carprofen should stop (hopefully 9/11). At the point of all pain meds stopped and carprofen stopped, only then will you have accurate information if there is still remaining pain.
