Hello there. My name is Mandi and my dog is Oscar. Oscar is a 13 year old dachshund. He had a bad bout with IVDD 4 years ago right around this time. He was completely down in the back then but fully recovered after conservative treatment. He had no issues for the last 4 years, but since then, developed IBD. He is currently on budesonide once a day for that, as well as metronitozole and sees and IMS. Yesterday, Oscar started acting oddly in the morning when he woke up. He had put himself into his crate and yelped when he was picked up. There was also some shaking. I immediately called out vet since they are open on Saturdays. We went in and they did a laser treatment and prescribed gabapentin. He was well all day Saturday. Over night, he took a turn for the worse. I created him and we took him to the emergency vet. They did another laser treatment and prescribed tramadol and said to continue the gabapentin. [Moderator's Note. Please do not edit weight? for IBD: budesonide for IBD, a corticosteroid but not absorbed systemically metronidazole for IBD for 1/5 disc episode gabapentin ? mgs ?x/day tramadol ? mgs ?x/day] They said not want to upset the balance of the IBD (it is bad) without discussing with the IMS. I know he needs to be on pred (which he was originally on before the budesonide) for the IVDD but that hasnt happened yet. He is not sleeping and does not seem to be comfortable. He rarely even lays down and hasnt slept. Any help is greatly appreciated. I'm nervous because we have work tomorrow.

Oscar is having a pain emergency.
He needs to have Prednisone to reduce the swelling in his spinal cord, Tramadol and Gabapentin 3x daily for the pain, methocarbomal for the muscle spasms, and pepcid ac to protect his stomach.
Right now Oscar has nothing addressing the swelling which causes pain and nerve damage.
He sounds very under medicated for pain.
Can you get Oscar to an emergency vet to get the right medications started ASAP?

I'm sorry to hear that Oscar is having another disc problem, Mandi. I hope when you consult with the vet who is treating the IBD today that they will come up with a solution whereby Oscar will be able to take an anti-inflammatory. Possibly the addition of Pepcid AC (Famotidine) and Sucralfate might be an answer but I'm not familiar enough with IBD to guide you.
What I can tell you is that Oscar's pain meds need to be adjusted ASAP to get his pain completely under control. Please call the vet ASAP this morning to advise of the signs of pain you are still seeing and advocate for an adjustment to the pain meds. Since we don't know the dosages of the pain meds currently given, we can't tell if the dosage are too low or not. Methocarbamol can be added to the mix - that med addresses the pain of muscle spasms that often accompany an IVDD episode. Pain should be completely under control within one hour of giving the new course of meds and should remain completely under control from one dose of pain meds to the next.
Please let us now how much Oscar weighs, the doses in mgs of each pain med prescribed and times per day to be given.
Weight?? Tramadol ??mg ??x/day Gabapentin ??mg ??x/day Methocarbamol added? anti-inflammatory added? additional stomach protection added?
