1 Does not appear to be in pain. He does limp when he first gets up. walks a bit slowly, but not so much when it isn't quite so hot out. There was shaking and crying with the initial incident, about a month ago. A bit of whimpering with the relapse on August 8. ☆ 2 45 pounds Gabapentin 200 mg 3 times a day Methocarbam 250 mg. 3 times a day He is on medication for hypothyroidism Prednisone 5mg. 3 times a day; start date first time July 26, 2021. Start date relapse August 9, 2021 upped to 10 mg. twice a day. Started to taper first time on July 31, seemed to have a relapse about August 9, Started to taper first time on July 31, seemed to have a relapse about August 9, started [Pred] back up on August 11 to 10mg twice a day, started stepping down August 18, seemed to be feeling worse on August 23 so upped it again, but a bout of diarrhea seemed to make him feel better so stepping it back down starting August 24. He jumped off an unfinished deck on July 18, started to appear in pain, yelped when turning his head on July 23. Could not reach vet but she called on Monday-wanted us to treat for herniated disk. He did very well (too well) for the first two weeks, and an escape and run incident, the weekend of August 7, and we had to start again with the crate rest. Vet doesn't recommend transporting him until he is off the medication due to risk of getting hurt by motion of the car. At 45 pounds we are at a loss for how to (1) get him into the car and (2) constrain him in the car (could do a crate, but what to do when we get to vet). ☆ 6 date you saw the vet: after the relapse, August 9. (telephone consult)☆ 7 He sometimes has crate accidents, but these are usually (not always) overnight. There are some days where he seems to have to pee and pee and pee-this is usually a morning issue and tapers off by around noon. He lifts his leg about half the time, keeps all legs down other times. Prior to crate rest and meds he would switch legs while peeing. ☆ 8 He can walk, although he is a bit slow and tentative-maybe wobbly at times, especially after waking up. Never been a big tail wagger, but he does raise it when he sees his people, bugs, birds, or rabbits.

The hallmark component of conservative treatment is the very STRICT crate rest part (no PT, little movement). With little blood supply, discs are much slower to form good scar tissue than it takes a blood rich broken bone to heal. Those weeks of a cast for a broken arm to heal is similar to the recovery suite being a kind of cast for the disc. 100% STRICT crate rest 24/7 for 8 weeks provides limited movement to allow good strong scar tissue to form. In Dozer's case, the 8 weeks would be from the date of the relapse. For constipation, try pumpkin. Pumpkin is a magical fruit - its high fiber can firm up stools and help with diarrhea or loosen the stool to help with constipation. Note alternatives: really ripe mashed fresh pear, just take off the peel off; microwaved and mashed peeled sweet potato. The amount of water in the diet makes all the difference. --To loosen the stool, add equal parts water to each kibble meal along with 1 teaspoon of plain canned pureed pumpkin for every 10 pounds of body weight, 1x a day. I tried pumpkin for my dog's constipation. It helped and he really liked it.
All anti-inflammatories cause excess stomach acid which can result in stomach damage. To prevent this damage, stomach protectors like Pepcid AC are used. The frequent burping may be helped by the Pepcid AC.
Steroids like pred make a dog want to drink more and therefore pee more. Dogs on pred may have to be taken out more frequently, sometimes as often as every two or three hours. For overnight accidents, try giving Dozer the last drink of the day a few hours before bedtime. Of course, he must have free access to water during the day.
If you can lift Dozer in his crate into the car that would be the safest way to transport him. The vet may have a stretcher or gurney available when you get to the office and a vet tech to help you get Dozer out of the car and into the office. When he is lifted, it is important to try to keep his back as straight as possible.
Is 5mg 3x/day the tapering dose he is currently on? Usually when the pred is tapered the pain meds are also cut back or stopped at the same time. This is because pain meds may mask any pain that becomes evident during the taper. Pain during the taper means that more time on the full dose of pred an all pain meds is needed.

Dozer paced incessantly when he hurt himself and when he relapsed. Vet felt it was due to pain, and he did cry/whimper occasionally at this time.
4 weeks out (two after the relapse) he seems to be feeling much better. However, quite often he paces in his crate after dinner. Never after breakfast. He will pace for around 20-30 minutes-off and on. I can tell him to sit and stay and he will for a bit. Around 6 he’ll settle down and sleep or look out the window until TV time at 9. Goes to bed around 11.
I don’t think he’s doing it because of pain as it’s always at the same time. It’s also when his pops gets home, and pops did lay next to him and pet him in the crate when he paced. For example-he paced and paced until pops just left for a haircut, was asleep within 5 minutes of pops leaving.
My question: can IVDD dog pace due to boredom? Apart from being a bit dazed and confused he seems in decent health and spirits otherwise.
I don’t want to over react (I know the vets office will likely have me increase his meds again), but don’t want to ignore s problem either. He’s always been an anxious/separation Anxiety kind of dog

Pam, did you get Pepcid AC (famotidine) on board as Romy mentioned yesterday? What are the names of these meds and the food fiber? name of hypthryoidsm meds name of dietary fiber 1x/day Dozer is doing too much movement of his back with that pacing. I do think you need to start with a stronger anti-anxiety med to get him relaxed in the recovery suite. You could also be giving some calmers a try to see if those would be enough once used for several days. Then possibly a transition full time to the callers would be all he needs. It is just a case of try and see what works best to keep him from pacing. USING CALMERS Using any oral calmer in combination with a Pheromone diffuser seems to work best. It takes several days for these to start working - it isn't immediate but they are a much better option if you can avoid heavy duty prescription meds (ACE, alprazolam or traZODone). Of course always keep your vet in the loop on all things you give your dog. Paula His thyroid med is thyrotabs .5mg The fiber is psyllium husks. I’ve given them to him before due to anal gland issues. I will try the diffuser. I asked vet before for anti anxiety but only got the receptionist -she went right to increasing the pred gabepentin and methacarbam. Unfortunately there are few vets out here and they are hard to talk to but will try to leave message specifically for the vet tomorrow. I have noticed a possible link between pacing and overnight diarrhea (single episode-always seems better the next day)
I have Pepcid but have not given him any as I’m not sure if the dose for his size

45 lbs Dozer X 0.44mgs famotidine = 19.8mgs famotidine. Round up to a full 20mgs famotidine tab and give every 12 hours (2x/day)
When you can, verify with vet Dozer does NOT have any health issues to not take Pepcid AC. Since he IS having diarrhea, may be a good idea to start now as it suppresses acids that Prednisone causes. How is Dozer today? Any signs of pain nearing next dose of pain meds (Gabapentin, Methocarbamol)?

Good Morning. Dozer was fidgety last night (again, when pops was with him) I noticed muscle spasms in his shoulder. He got his medications and was asleep within a half hour. He woke up normally this morning (appears to have slept through the night); however, is limping a bit with the same shoulder (right after getting up from a nap). He's settled (not pacing) this morning. Eating and drinking well. Pooped and pee'd before bed last night; however, did pee in his crate at some point overnight.
He seems to get very excited when his pops is around (pops has anxiety too), but settles quickly when all is quiet. I've put an x-pen out on the grass rather than have him walk around.
I stopped the fiber, will pick up pumpkin today. He should be on one 5mg a day pred after today. I have been adjusting the methacarbam when he seems to have spasms, then trying to drop the dose back down.

Pam, the idea with using pain meds with a disc episode is to be consistent VS up and down with only when pain appears. When pain surfaces then you are too late! Always assume a painful disease is painful and give the meds promptly on time every 8 hrs as prescribed (not on a up and down basis every several days or so). In general the only time to lower meds (usually it would be a full stop of pain meds) is on the date Prednisone taper starts. The taper is the time to test for pain. Dozer IS currently in a taper now at 5mgs 3x/day. -- You report signs of pain (muscle spasms in his shoulder and limping with the right shoulder. -- You are the eyes and ears at home for your vet and need to quickly report signs of pain and any new or increased neuro function loss. -- With your report of pain, then clearly it would not be time to continue with the taper going to 8/28 5mgs Pred 1x/day. -- Instead your vet would very likely go back up to the original anti-inflammatory Prednisone level dose of 10mgs 2x/day. Your vet might say a 7-day course (and then try another pred taper at the end of the 7 days to test for pain). Did your vet say Dozer had a neck disc or did he suspicion the disc to be in the back? So I gave him composure. It is now 2:30 am and the dog is unrecognizable. Panting. Hyperventilating. Trying to run away. Horrible. I don’t know if he will survive this

Pam, Composure can take several days to build up before it works to calm. It also works best with used along with a DAP fall plugin diffusor. What date did you start giving Composure?You reported pain on Friday 8/27. Did the prednisone taper continue on 8/28 to a further lower to 5mg 1x/day?
Panting can be a sign of pain OR a sign of anxiety OR can possibly be related to prednisone use. If pain, you would very likely also see other signs of pain besides the shoulder muscle spasm and limping with from leg. On the taper of prednisone, this means it would not be time to continue with the taper doses of going down to 5mgs 1x/day scheduled for 8/28. Your vet would likely go back up to the original Prednisone anti-inflammatory level dose. Were you able to contact your vet on Aug 27 and report pain?If panting is related to Prednisone use: -- Try a fan near crate but not pointed at the dog to help circulate air some. -- Try a frozen broth ice cube to lick on. Dr. Isaacs discusses this issue: www.dodgerslist.com/neurocorner2/panting.htmIf related to stress, anxiety, then first he may need the vet's help of a prescription med. Composure works best when also used with a DAP plug in diffusor. When the combo DAP + Composure has had days to build up, then experiment by stopping the vet Rx sedative to see if the two calmers are enough to help Dozer relax at night and during the day. If not, then Dozer may just need to continue on with the vet's anxiety medication. When your brain is calm and rested and Dozer is OK, please do paint a full picture for us with all the details of dates, his current pain situation, any changes to meds.

I got the diffuser on Friday. Gave the composure around 7pm on 8/28. His normal routine was to look at me when I got up, watch me make coffee and get his pills and breakfast ready. Feed him and take him out. He would nap off and on, getting his second pills at 3:30, then watch mil til dinner at 4:30. After dinner he would be anxious, sometimes pacing, until around 7 when he dozed off again. Was sleeping thru the night
Yesterday we were doing some construction (fencing on the concrete patio) outside the door he looks out of. He was more antsy than usual, and didn’t settle down as usual. Gave him the composure and he napped for a bit but back awake around 9:30 and got very active. Managed to wipe him with a cool cloth and put ice on his back and he napped until midnight (taking final meds at 10:30). Would not settle back down. We ended up putting a mattress on the floor and taking turns holding him so he didn’t hurt himself in his crate stumbling around. He also stood in the corner of the crate panting heavily. He slept maybe from 4-6, then back up with panting and trying to pace. This went on until maybe 9:30 am when I gave him another composure
Called the ER vet and the person asked what was going on. She suggested waiting to see how he felt after his nap. That’s where we are now.
Did not eat much breakfast this am and didn’t drink. I did step down his prednisone yesterday.

So update. Around 2 pm 8/29 he woke up, had a big drink, ate the rest of his breakfast, went outside to hang in hi x pen a bit. Enjoyed an ice pack on his back fell asleep. Woke up around four pacing panting and had pooped the crate. Wouldn’t drink. I put a compose in his mouth and he ate it-seems to have settled a bit but I’m afraid he will hurt himself more if I try to hold him so I put him back in his cleaned crater

How was he poop, constipated or normal. Is he on plain pureed pumpkin as of 8/27 of 4 1/2 teaspoons plain canned pureed pumpkin 1x a day to kibble that has extra water added to his freeze dried meal?
Normally when pred is being tapered all the pain meds are stopped so at home it is quick to see if pain resurfaces and easy to accurately observe about pain. Maybe having methocarbamol and gabapentin on board is making is hard for you to be accurate about attributing muscle spasms/limping to pain. With your vet open on Monday, let us know what you both conclude is going on with Dozer. ( --- Does he need something like traZODone for anxiety --- is he he in pain which indicates the need to go back up on Pred
Just so many things going on that your vet's knowledge of Dozer and his veterinary knowledge may be the combo to figure out what's going on.
Is hanging out in a hi expen outdoors mean the space is limited to only being able to stand up on all four legs, having the ability to easily turn around and if lying down able to fully stretch out his legs? In other words not really the ability to pace in his outdoor recovery suite.

Poop is normal. His crate is 2ft by 3ft so you wouldn’t think he could pace in it, but he is. I’ve called two emergency vets -both suggesting roughing it out until tomorrow.
Meanwhile, panting and pacing nonstop. Nearest vet open on a Sunday around her is an hour away

Pam, you don't want to reward him for doing unwanted behavior. So..... see if you can get him to follow a command such as "sit", where you can then reward him with a tiny treat and praise him. Then allow him out onto an adjacent-to-the crate, bed or mattress. Sit with him til he calms. Hopefully he would fall asleep? Then when he is sleeping someone can bring in the tall ex-pen from outside. Wrap the panels around the mattress (folding back accordion style the panels not needed to the side) making a limited movement recovery suite. MUSIC Try some calming music. Here is 6 hours of relaxing music for anxiety: Hope to hear you are successful in getting him to stop all his movement of pacing and tight turns he must have to be doing. Drape a blanket to provide a more cozy-like den area for Dozer Is he on plain pureed pumpkin as of 8/27 of 4 1/2 teaspoons plain canned pureed pumpkin 1x a day to kibble that has extra water added to his freeze dried meal? No longer taking the dietary fiber stuff?

Dozer was put to sleep at 11am this morning. Vet believed spinal or brain tumor.

Oh, Pam my heart is so very sad to hear this unexpected news. My deepest sympathy for the loss of your dear Dozer.
